Significance Of Anti-?-fodrin Antibody In The Diagnosis Of Sj(?)gren''s Syndrome

Background: Sj?gren's syndrome(SS)is an immune and genetic and other factors associated with autoimmune diseases,including the xerostomia and dry eye disease and salivary glands,as well as the involvement of the lung,renal interstitial lesions,nerve lesions and cutaneous vasculitis.Diagnosis relies on clinical manifestations and labial gland biopsy and other invasive examination,there is no ideal method for serological diagnosis.The positive rate of anti Ro/SSA antibody and anti La/SSB antibody are 60~70%,25%~50% in Sj?gren's syndrome.Due to its low sensitivity and specificity,many scholars continue to explore new autoantibodies.Mary studies found,?-fodrin(Fodrin)is closely related to SS.Objective: To investigate the the significance of anti-?-fodrin antibody IgA and IgG in the diagnosis of Sj?gren's syndrome and its correlation with clinical manifestation.Methods: Recombinant human anti-?-fodrin IgA and IgG was used as envelope antigen in enzyme-linked immunosorbent assay(ELISA)to detect the relatively specific antoantibody in sera of 108 primary SS,187 rheumatoid arthritis(including51 secondary Sj?gren's syndrome,sSS),54 systemic lupus erythematosus(including10 sSS),8 systemic sclerosis,6 polymyositis,3 dermatomyositis,5 multiplesclerosis and 98 health between Oct.2012 to Aug.2015.Result:(1)Sensitivity of combined Anti-?-fodrin antibody,anti SSA antibody and anti SSB antibody is high than separately(P < 0.05).(2)Sensitivity of combined Anti-?-fodrin antibody is better than anti SSA antibody,anti SSB antibody in pSS(P < 0.05).Meanwhile,no significant difference between comblined three kinds of antibody detection.(3)The positive rates of IgA-Fodrin and IgG antibody in serum were 78% and 40.8%respectively,which were significantly higher than those of SLE,RA and other connective tissue diseases,and the difference was statistically significant(P < 0.05).(4)The positive rate of anti-Fodrin IgA antibody in pSS was higher than that of sSS,and the positive expression of IgA in pSS was higher than IgG(P < 0.05),and the positive detection rate of anti IgG-Fodrin antibody in pSS and sSS was notstatistically significant.(5)The positive expression rate of anti IgG-Fodrin was higher than anti IgA-Fodrin in SLE and RA(P < 0.05).(6)The positive rate of anti-?-fodrin antibody was more than 20% in SS patients whose serum anti SSA antibody and anti SSB antibody were negative.Conclusions: The results showed that anti-?-fodrin antibody as a diagnostic marker of SS,especially for early diagnosis of SS.pSS patients with anti-?-fodrin IgA antibodies was more common than the presence of anti-?-fodrin IgG antibodies,and the positive rate of anti-?-fodrin IgG antibody was higher than anti-?-fodrin IgA antibody in RA,SLE patients.Comblined with anti-?-Fodrin antibody,anti-SSA and anti-SSB antibodies is a new diagnostic marker for patients with SS,anti-?-Fodrin antibody had high specificity.It is helpful for the diagnosis of SS especially for patients with negative anti-SSA and anti-SSB antibodies.
