| Objective: To study the pathogenesis factors related to the onset of Mi-nimal Hepatic Encephalopathy(MHE)by collecting the incidence of liver cirrhosis patients with MHE,to research the intervention effect of Rhubarb Decoction on MHE patients’ Traditional Chinese Medicine symp-toms score,serum endotoxin content,aptitude tests score,quality of lif e and other indicators.Methods:1.To collect the incidence of MHE by testing 142 liver cirrhosis patients with the Number Connection Test-A(NCT-A)and Digit Symbol Test(DST).To analyze the relationship between the incidence of MHE a-nd ascites,serum ammonia,total bilirubin,prothrombintime,serum albu-min,Alanine aminotransferase,Aspartate transaminase,γ-glutamyl tran-speptidase,alkaline phosphatase and liver function Child-Pugh score.2.To select 60 liver cirrhosis patients with MHE witch were randomly divided into experimental group(Rhubarb Decoction group)and the control group(lactulose group)at a ratio of 1: 1.Two groups of patients were treated by corresponding drugs retention enema for seven days,twice a day.Also two groups of patients were tested by NCT-A、DST,quality of life assessment BREF,symptom scores,electroencephalogra(EEG)and EEG topographic maps,serum endotoxin detection to evaluate the Intervention Effect of Rhubarb Decoction on MHE.Result:1.This study included 142 cases of liver cirrhosis,of which 66 pa-tients were detected positive by NCT-A and DST.The prevalence rate of MHE is 46.5%.It was showed a positive rate of MHE with Child-P ugh grade B and C,ascites positive,lower serum albumin,prolonged p-rothrombin time,elevated serum ammonia was higher than that of decompensated liver function,ascites tumors,normal serum albumin,norma l prothrombin time and normal Serum ammonia(P<0.05).The regression analysis showed a significant correlation between MHE and ammonia,Child-Pugh score,ascites,serum albumin,prothrombin time(P<0.05).Aslo It was showed there was no major relevance between MHE and aspart-ate aminotransferase,total bilirubin,alanine aminotransferase,γ-glutam yl transpeptidase and alkaline phosphatase(P>0.05).Comparing the quality of life scores of MHE and non-MHE patients,the difference was stati-stically significant(P<0.01).2.Comparing the result of test by NCT-A,DST,quality of life ass-essment BREF,symptom scores,EEG and EEG topographic maps,seru m endotoxin detection before and after Rhubarb Decoction retention ene ma,the difference was statistically significant(P<0.05).Comparing the symptom scores of Rhubarb Decoction group and the lactulose group,th e difference was statistically significant(P<0.05).But the difference of o ther indicators was not statistically significant(P>0.05).Conclusion:1.There was a significant correlation between MHE and ammonia,Child-Pugh score,ascites,serum albumin,prothrombin time.All the ev-ated serum ammonia,higher Child-Pugh score,ascites,lower serum alb-umin and prothrombin time were the risk factors of MHE.Quality of life in patients with MHE was significantly reduced than that in non-MHE patients.2.The treatment of Rhubarb Decoction retention enema has a goo d Intervention effect on MHE witch could shorten the time of NCT-A,increase scores of DST,improve quality of life,improve clinical symptoms,improve the abnormal EEG and EEG topographic map,reduce ser-um endotoxin concentration,restore liver function.Also the improving of symptom scores with Rhubarb Decoction group was significantly reduced than that in lactulose group.But the improving of other indicators were the same efficacy. |
