Objective:To investigate the value of diffusion weighted imaging?DWI?in antidiastole of prostate cancer and benign prostatic hyperplasia.And to research the effect of different b values for measurement results of ADC in prostate cancer and the relationship of prostate cancer between ADC score and pathological Gleason score.Also to explore the quantitative parameters of Ktrans,kep,Ve and Vp for dynamic contrast-enhanced MRI?DCE-MRI?in antidiastole of prostate cancer and benign prostatic hyperplasia and its correlation with pathological Gleason score.Methods: 32 cases of prostate cancer patients and 35 cases of benign prostatic hyperplasia patients in our hospital from November 2014 to December 2015 in Department of Urology are collected.All of them have been underwent conventional MRI sequence,multi b value DWI sequence,multi flip angle sequence and DCE-MRI sequence.We compare the differences of ADC score for prostate Cancer in the condition of different b value and the differences of ADC score between benign prostatic hyperplasia and prostate cancer.We use Omni Kinetics software to postprocess the DCE data.We compare the difference of Ktrans,kep,VE,VP in prostate cancer and benign prostatic hyperplasia and cancer and non cancer district area and compare them with the Gleason score and ADC score.Results: 1.When the b value is 700s/mm2,1000s/mm2,1500s/mm2,the ADC scores of prostate cancer size respectively?0.876±0.192?mm2/s,?0.809±0.179?mm2/s,?0.697±0.155?mm2/s,and the difference between the former and latter have statistical significance,but the difference between the first and second have no statistical significance.ROC curve analysis showed that three group B value of the area under the curve for differential diagnosis of prostate cancer and benign prostatic hyperplasia are 0.986,0.988,0.992.b value of 1500s/mm2 best the diagnosis threshold 1.026 × 10-3mm2/s,the sensitivity and specificity proswere 92.9% and 100%.2.When the b value is 1500s/mm2,the ADC score of prostate cancer,prostatic hyperplasia central area and tatic hyperplasia peripheral zone size respectively?0.701±0.162?mm2/s,?1.265±0.116?,mm2/s,?1.564±0.261?mm2/s.And there were statistically significant differences between the three.3.The Ktrans,kep,Ve and Vp value of Prostate cancer size respectively?1.310±0.737?/ min,?1.663±0.490?/ min,0.763±0.385,0.148±0.138.Prostatic hyperplasia central area parameter values size respectively?0.962±0.720?/ min,?128±0.435?/ min,0.777±0.416,0.054±0.074;prostatic hyperplasia peripheral zone parameter values size respectively?0.423±0.453?/ min,?0.757±0.418?/ min,0.603±0.417,0.034± 0.034.The differences of Ktrans and kep in the comparison of prostate cancer and benign prostatic hyperplasia in each group have statistically significant,there was statistically significant difference between benign prostatic hyperplasia and prostate cancer by Vp value.The differences of Ve value in the comparison of each groups have no statistical significance.ROC curve analysis the area under the curve of Ktrans,kep,VP in the diagnosis of prostate cancer and benign prostatic hyperplasia were 0.771,0.805,0.830.Ktrans diagnostic thresholds for 0.893/min.The sensitivity and specificity respectively for 71.9%,78.7%.Kep diagnostic thresholds for 1.318/min,the sensitivity and specificity were 81.3%,70.5%.Vp diagnostic thresholds for 0.055 the sensitivity and specificity were 78.1%,78.7%.4.The ADC score of prostate cancer was negatively correlated with Gleason score.The DCE quantitative parameters have no significant correlation with Gleason scores.Conclusion: 1.Different b values have a certain impact on the measuring results of ADC value for prostate cancer.The higher b value of prostate cancer by measuring,the ADC value lower,the more accurate the results.When the b value is 1500,the ADC value measurement result is more reliable.2.DWI have a certain value in the differential diagnosis of prostate cancer and benign prostatic hyperplasia.The ADC value can well distinguish benign prostatic hyperplasia and prostate cancer.The ADC value of cancer is lower than that of proliferation.3.ADC of prostate cancer was negatively correlated with pathological Gleason score.Malignant degree by measuring ADC value can reflect the tumor in a certain extent.4.DCE-MRI has a certain value of quantitative parameters for diagnosis and differential diagnosis of prostate cancer.Ktrans and kep can distinguish prostate cancer,benign prostatic hyperplasia?BPH?central gland area and hyperplasia of prostate and peripheral zone,the VP can distinguish between benign prostatic hyperplasia and prostate cancer in a certain extent,but in distinguishing benign prostatic hyperplasia central gland area and hyperplasia of prostate and week take effect is poor.The measurement of the corresponding parameter values for the corresponding measurement of prostate cancer were higher than the value of the prostate hyperplasia.The measurement area of central gland hyperplasia hyperplasia were higher than peripheral zone measurements.The value of Ve in differential diagnosis of prostate cancer and benign prostatic hyperplasia is relatively low.5.Prostate cancer DCE quantitative parameter Ktrans and pathological Gleason score no significant correlationIn short,DWI and the ADC values can very good points of prostate cancer and benign prostatic hyperplasia,and to a certain extent reflect the malignant degree of prostate cancer;DCE quantitative parameters Ktrans and kep can well discriminate between prostate cancer and benign prostatic hyperplasia district.VP is also a certain value in the differential diagnosis of prostate cancer and benign prostatic hyperplasia.
