Clinical Observation Of The Influence About The Outcome On Bushenyangmo Decoction Do For Kidney Defficency Patients In The Natural Cycle Of Frozen-Thawed Embryo Transfer

Objective:To observe the therapeutic effect of the traditional Chinese medicine called Bushenyangmo decoction for Kidney defficency patients who have chosen the natural cycle of frozen-thawed embryo transfer(FET) in assisted reproductive technologies(ART),and to provide the feasible advice for them from the viewpoint of traditional Chinese medicine(TCM).The objective of this thesis is also to made it clear that Bushenyangmo decoction can increase the outcome of pregnancy by improving the endometrial receptivity of that patients,which provide effective treatment scheme and basis for TCM to improve its pregnancy rate in assisted reproductive technology.Methods: According to the sequence of the cases inducted into the study to number 80 patients, who conforms to the Diagnostic criteria and TCM kidney deficiency dialectical standard were divided into the controlled group 40 cases and the tested group 40 cases in accordance with random number table. In comparison with the controlled group, the tested group not only take the medical treatment with the conventional natural cycle of FET program, but also be given Bushenyangmo decoction from the second day of menstruation to embryo transfer day. Observing the improvement of symptoms of TCM before and after treatment, endometrial thickness and endometrial blood type, the endometrial blood flow in Applebaum type, Pulsation index(PI) and Resistance index(RI) of uterine spiral artery blood flow on the dominant follicle day, and the related endocrine hormones on the dominant follicle day and transplantation day, and endometrial thickness on the transplantation day. And comparing and analysing the differences about the patients of two groups in cycle cancellation rate, embryo implantation rate, ectopic pregnancy rate, clinical pregnancy rate and live birth rate.Result:Compared with the controlled group, the tested group improved significantly inkidney deficiency syndrome(P=0.000).In comparison, the endometrial blood flow in Applebaum type of the tested group was better than the controlled group on the dominant follicle(P<0.05).The tested group was significantly better than the controlled group in the comparison of E2, PI and RI of uterine artery blood flow and endometrial thickness on the dominant follicle day(P=0.000) and in the estradiol(E2) levels and estradiol and progesterone level ratio(E2/P) on the transplantation day(P=0.000). There are 1 cases in tested group and 8cases in controlled group have no chance to be transferred because of the thinner endometrial thickness, and the cancellation rate of the tested group(2.56%) was lower than the controlled group(21.05%)in comparison of the cycle cancellation(P<0.05). Embryo implantation rate(the tested group: 73.68%, the controlled group: 46.67%), clinical pregnancy rate(the tested group: 65.79%, the controlled group: 40%) and live birth rate(the tested group: 39.47%, the controlled group: 16.67%) in the tested group were higher than the controlled group(P<0.05).And there was no significant difference in the rate of ectopic pregnancy(the tested group: 2.63%, the controlled group:3.33%).ConcIusion:1 Bushenyangmo decoction can significantly improve the patient's kidney deficiency symptoms;2 Bushenyangmo decoction has the effects like estrogen, which can improve the level of E2 on the dominant follicle day, the estradiol( E2) levels and estradiol and progesterone level ratio(E2/P) on the transplantation day. In addition, it can also increase the thickness of the endometrium through the improvement of endometrial perfusion, and so as to improve the endometrial receptivity;3 Bushenyangmo decoction can reduce the cycle cancellation rate and improve the embryo implantation rate, clinical pregnancy rate and live birth rate by improving the endometrial receptivity.
