Objective To observe the effect of Shenzhutiaopi Grain(SZTP) with Yiqi Jianpi Huashi effectiveness on blood glucose, blood lipid and insulin levels in IGR patients,and the effect of the IGR reversal rate. At the same time, to study the effectt of SZTP on serum fasting insulin levels, Nrf2, and HO-1 expression levels in rats from the protein factor level.To investigate the effect of SZTP with Yiqi Jianpi Huashi effectiveness on the mechanism of action for insulin resistance and insulin sensitivity in SD rats.Methods 1 Clinical Research Research objects:The 83 cases of pre diabetes patients were randomly assigned to treatment group(42 cases) and control group(41 cases), were given based lifestyle intervention, including diabetes health education, diabetes diet control guidance, diabetes and exercise guidance; treatment group on the basis of intervention and particles of SZTP, twice a day, one bag each time, rinse. The courses of the two groups' treatment were 12 months and three months for a course of treatment, a total of four courses. Methods: Calculating the two groups of before and after treatment patients with total efficiency and TCM syndrome integral, blood glucose, lipids, insulin, glycosylated hemoglobin level changes and calculation on insulin resistance index HOMO-IR and insulin sensitivity index(ISI) changes.2 Experiment Study subjects: 60 male healthy SD rats were randomly divided into blank group 10 and 50 rats after feeding for 1 weeks.The blank group were fed with normal diet, and the model group was fed with high fat diet.After 12 weeks, the blood was taken from the tip of the tail, and the fasting blood glucose and the glucose by O GTT experiment were detected in 2 hours after.A total of 40 rats were successfully created.It was randomly divided into 3 groups: model group 13, low dose group 13 and high dose group 14.The blank group and model group were given normal saline.the treatment group was also given the different dose of SZTP. The medicine was given to the stomach for 4 weeks. At the end of the experiment, the number of rats was: 9 in the blank group, 11 in the model group, 12 in the low dose group and 12 in the high dose group.Research methods: Before and after drug administration, rats were observed the behavior, weight, and other general situation.16 weeks later, the rats were anesthetized and dissected, blood was taken from the abdominal aorta, FPG, FINS, TG, TC, HDL-C, LDL-C and other indicators were detected.At the same time, using the Western blot method to detect Nrf2 from liver tissue, the expression levels of HO-1.Results 1 Clinical Research Clinical observation showed that after 12 months of SZTP treatment, Two groups before and after treatment, the difference of Chinese medicine syndrome score is significant(P<0.01), the treatment group compared with the control group, the difference was significant, with statistical significance(P<0.01);The total effective rate of the control group is 60.97%, the treatment group is 80.95%, the treatment group is better than the control group(P<0.05); Compared with the control group,the symptoms of fatigue and physical difficulty in the treatment group improve Obviously(P<0.05). After 12 months of treatment, the BMI of the control group and the FPG, 2h PG, Hb A1 of the treatment group are significantly different between before intervention and after the intervention(P<0.05, P<0.01).In the treatment group, the 2h PG, Hb A1 c after treatment compared with the control group, there are significant reduced(P<0.05). The control group TC level between after and before treatment is statistically significant(P<0.05), treatment group TG, TC, HDL-C, LDL-C are statistically significant(P<0.05 or P<0.01) before treatment.The treatment group compared with the control group, the improvement of TG and TC are statistically significant(P<0.05 or P<0.01).After treatment, the control group FINS,2h INS increase than before(P<0.05), treatment group FINS, 2h INS than before the intervention significantly reduced(P<0.01);After the intervention, the FINS,2h INS of the treatment group compared with the control group, the difference is statistically significant(P<0.05). There isn't statistically significant difference between and a fter treatment of the control group IR, ISI(P>0.05);However, IR in treatment group is significantly lower than before treatment, ISI is significantly higher than before treatment, all ha ve statistical significance(P<0.01);Compared with the control group, the treatment group IR decreased, ISI increased, the differences are statistically significant(P<0.05).After 12 months of treatment, 10 patients(24.39%) in the control group convert to normal blood glucose and 14 patients(34.15%) become T2 DM in 41 patients;In the treatment group, 25 patients(59.52%) of 42 patients converte to normal blood glucose, and 6 patients(14.29%) become T2 DM. The rate of normal blood glucose of the treatment group is significantly higher than control group. The rate of T2 DM of the treatment group is significantly lower than control group(P<0.05).2 Experiments Animal experimental results show that the model is successful, the model group compare to the blank group, weight increased significantly(P<0.01).After treatment, model group, low dose group, high dose group rats weight compared with the blank group, the difference is significant(P<0.01).In low dose group, high dose group compared with model group weight loss significantly, there is statistical significance(P<0.01).After successful modeling, the blood glucose of rats in the model group is significantly higher than that in the blank group(P<0.01).After treatment, the blood glucose of low and high dose group are significantly lower than before(P<0.01).Low, high dose group and model group are significantly lower blood glucose(P<0.01), and low, high dose group, high dose group of blood glucose is significantly lower than the low dose group, there is statistical significance(P<0.01).After treatment, TG, TC, HDL-C, LDL-C levels in the model rats are statistically significant compared with the blank group(P<0.01).Low, high dose group TG, TC, HDL-C, LDL-C levels are significantly different compared with the model group, there is statistical significance(P<0.01).In high dose group TC, TG level are significantly lower than that of low dose group, and there is statistical significance(P<0.01).After treatment, model group and low, high dose of FINS, HOMA-IR are significantly higher than the blank group, ISI is significantly smaller than the blank group, the difference is significant(P<0.01).The low, high dose group HOMA-IR, FINS compared with the model group significantly decrease significantly, ISI significantly increased(P<0.01).The low, high dose group FINS, HOMA-IR, ISI comparison also has significant difference, there is statistical significance(P<0.01).significance(P<0.01).After treatment, the model group and drug group rats' liver tissue Nrf2, HO-1 protein expression levels are significantly higher than that in the blank group(P<0.05, P<0.01).The expression level of HO-1 in high dose group is significantly higher than that in model group and low dose group(P<0.01).Compared with model group, the low dose group has no significant difference, and there is no significant difference(P>0.05).Conclusion SZTP can significantly improve the symptoms of qi deficiency and dampness stagnation in patients with diabetes, can effectively reverse the impaired glucose regulation, so that the early stage of diabetes to normal, or effectively delay the development of diabetes and diabetes.The mechanism is: reduce the level of blood glucose, the upregulation of hepatic Nrf2 and HO-1 protein expression levels, through the regulation of Nrf2/HO-1 pathway reduced cellular oxidative stress, so as to improve insulin resistance, improve insulin sensitivity, protect islet function.
