Objective: The aim of study is to observe the effect of Danzhi Jiangtang Capsule(DJC) on expression of TNF-? and CXCL-5. Meanwhile, to observe other factors which are related to vascular lesion, such as CTRP-9 and Hcy. To explore the correlation and pathogenesis between inflammatory indices and Type 2 Diabetes Mellitus.To investigate the possible mechanisms of DJC on vascular endothelail tissue and inflammatory reaction in diabetic patients and experimental rats.Methods:1. Clinical Research:60 diagnosed diabetic patients and 30 healthy subjects were selected from the first affiliated hospital of Anhui University of traditional Chinese medicine.According to the random number method, the diabetic patients were divided into control group and treatment group:control group has 30 cases and the treatment group was 30 cases.The treatment group was given conventional treatment(normal control group,) while another group was given both conventional treatment and DJC(treatment group) for 12 weeks.The FPG, 2h PG, Hb A1 c, insulin, C-peptide,blood lipids, blood rheology indicators are measured before and after treatment. The levels of CTRP-9 and Hcy were tested by enzyme linked immunosorbent assay(ELISA).2. Experiment:10 rats were randomly selected as control group from 55 healthy rats fed with normal diet.While the other called diabetes mellitus group were fed with high fat diet.After 4 weeks, rats in diabetes mellitus group were injected with streptozotocin(STZ) except control group.72 hours later, rats were tested blood glucose, the successful model of the rats were randomly divided into four groups: model group(n=12), pioglitazone group(n=11,rats treated with pioglitazone 10mg/(kg·d)), high dose group of Danzhi(n=11,rats treated with DJC 1.08g/(kg·d))and Danzhi low-dose group(n=11,rats treated with DJC 0.54g/(kg·d)).While the control group and diabetesmellitus group receive gavage of normal saline in 5m L/(kg·d) for eight weeks.At the end of experiment, the animals were sacrificed. Trunk blood samples were collected to measure FPG,TC, TG. The level of TNF-? and CXCL-5 was measured by ELISA.Results:1. Clinical Research:The clinical study found that the total efficiency of treatment was 85.7% and 55.2%in treatment group and control group,the curative effect in treatment group is better than control group(P<0.05).After treatment, the levels of FPG,2h PG,FINS,2h INS,FC-P, 2h C-P, and Hb A1 c in control group and treatment group was decreased significantly compared with pre-treatment(P<0.01), but the difference between the two groups was not obvious(P>0.05).The level of TG,TC and LDL in control group and treatment group was decreased compared with pre-treatment(P<0.01),and after treatment, the level of HDL in treatment group was significantly improved compared with control group(P<0.05). After treatment, the level of CTRP-9 in two groups was increased significantly compared with pre-treatment(P<0.01),while the level of Hcy was decreased compared with pre-treatment(P<0.01),the difference between the normal control group and treatment group had significance(P<0.05).2. Experiments:Appetite, drinking, the levels of FPG and blood lipid of rats in model group,pioglitazone group,high dose group of Danzhi and Danzhi low-dose group are higher than those of normal control group(P<0.01). Appetite and drinking of rats in group Danzhi decreased much more than model group after treatment(P<0.01).The level of blood glucose in the model group were lower compared with the in the normal control group(P<0.01). After the drug treatment,the level of blood glucose in the DJC was significantly decreased(P <0.01), and its effect is similar to the pioglitazone group. The levels of TG and TC in pioglitazone group and Danzhi group was decreased compared with the model group(P<0.01).Compared with the normal control group,the expression of TNF-? and CXCL-5 in model group was significantlyincreased(P<0.01),after giving Danzhijiangtang Capsule,the high expression of two groups was decreased significantly(P<0.01).Conclusions:Danzhi Jiangtang Capsule could markedly improve the clinical symptoms and the inflammatory indices. The possible mechanisms could be:to regulate glucose and lipid metabolism;to improve insulin resistance;to reduce the levels of CTRP-9 and Hcy while decrease the expression of TNF-? and CXCL-5.
