| Obective Discuss the effect of dexmedetomidine on the tolerance of tracheal tube during emergency in the postanesthesia care unit(PACU)and the quality of postoperative at 24 h after surgery.Methods Ninety patients(ASAⅠ~Ⅱ)scheduled for Radical operation of thyroid cancer were randomly divided into 3 groups.There were no DEX used in control group(group C)during the anesthesia and PACU;0.8ug·kg-1 of DEX were used in group D within 10 min as a bolus,and 0.4ug·kg-1·h-1 of DEX were infused continuously until the time of extubation;0.4 ug·kg-1 of DEX were used in group E within10 min only when the patients were send to the PACU.Recorded the time of emergency,the status of the tolerance of tracheal tube and the hemodynamics;The concentration of plasma hydrocortisone was examined in preoperative,the end of operation and after extubation;And measured the quality of recovery after surgery with the 40-item quality — of — recovery sowing system(QOR-40).Results The percentage of complete intolerance of tracheal tube when emergency were smaller in group D and group E when compared with group C(P<0.05);The plasma cortisol concentration in group D was decreased at the end of operation,and after extubation,both group D and group E were decreased(P<0.05);During the emergency,The hemodynamics of patients of both group D and group E were more stable when compared with group C(P<0.05);The group D has more scores in physical comfort,physical independence,pain and total scores(P<0.05);The time of emergency was no difference in group D and group C.Conclusion DEX applied in Radical operation of thyroid cancer during the anesthesia until the time of extubation in PACU can reduced the dosage ofanesthetic,improved the the tolerance of tracheal tube during emergency in PACU and the quality of postoperative at 24 h after surgery and diminished the fluctuation of hemodynamics,without significant influence on emergency time.It was usable to use single dose DEX in PACU to improve the the tolerance of tracheal tube,but the effect on the quality of postoperative is not obvious. |
