Objective: Sepsis is a thorny issue that ICU workers often faced,Secondary to multiple organ failure(multiple organ dysfunction syndrome,MODS)has become the leading cause of death in critically ill ICU patients.Studies show that the Proanthocyanidins have a broad anti-inflammatory effect,and the proanthocyanidins is widely present in people's daily diet polyphenols.But its mechanism is not yet conclusive.APACHE?has been widely used in the word,but the domestic ICU temporarily throw in a backward state,In this study,by collecting our emergency ICU patients with sepsis,ratings were APACHE?,explore the relationship between their scores and mortality.In cell experiment,Using LPS-induced THP-1 cells,sepsis,with or without PI3 k / akt pathway inhibitor LY294002,coupled with the intervention of proanthocyanidins,comparison of the expression levels of the different treatment groups NF-KB activity and TNF-??IL-6.To investigate the mechanism of proanthocyanidins.Methods: Clinical Investigation: Collecting ICU patients with sepsis received hospital,Remove patients younger than 15 years of age,less than 24 hours of admission and transfer out of ICU,do APACHE? rating,Discuss the relationship between the value of mortality in patients with sepsis and septic prognostic value of ICU patients.Cell Experiment: THP-1 cells cultured in vitro with 10% fetal calf serum + RPMI-1640 medium as the culture environment for 37 ?,5% CO2.Cells grown to log phase(concentration 4-8 × 105 / ml)can be subcultured and drug testing Choose safe and effective administration of proanthocyanidins concentration,excluding the impact of the results of drug toxicity using cck-8.Grouping controlled by adding appropriate amount of procyanidins B1 intervention sepsis model(LPS-induced THP-1 cells)with or without the addition of PI3 K / Akt inhibitor LY294002.Determination of NF-KB activity using Western-blotting and ELISA method assay concentrations of TNF-??IL-6.Reveal proanthocyanidins ease LPS-induced inflammation and PI3 K / akt / relationship nf-kb pathway,Collect experimental data results,And the use of SPSS 22.0 statistical description,mean ± standard deviation(± s)described statistics ANOVA statistical analysis of the test group differences when P <0.05 was considered statistically significant.Results: APACHE? score and ICU patients with sepsis mortality rate was positively correlated.Under CCK-8 toxicity testing laboratories.Found that procyanidins B1 at a concentration less than or equal 100?g / can not cause toxic effects on the viability of THP-1 cells,when the case of the concentration of procyanidins B1 is greater than 45ug/ ml(eg 90 ug / ml,180 ug / ml)THP-1 vitality will decline.So we select procyanidin B1 concentration of 45ug/ml as the optimal dosing concentration.In the relative concentrations of Western-blotting experiments NF-?b dynamic testing and the use of protein detected by ELISA in the different experimental groups of TNF-?,It found that procyanidins B1 can significantly inhibit the expression of inflammatory cytokines in cell model of sepsis.The addition of the cell model of sepsis PI3 K / AKT inhibitor LY294002,further expression of TNF-? and NF-kb is inhibited.Conclusion:1.APACHE? score was positively correlated with ICU mortality in patients with sepsis,and it is important to the sepsis patients' prognosis.2.Procyanidins B1 of THP-1 cells in a concentration-dependent toxic relationship,the concentration of procyanidins B1 greater than 45 ug / ml when the THP-1 cell viability would adversely affect.3.Proanthocyanidins inhibit LPS-induced inflammation.4.Procyanidins is B1 and PI3K/AKT-nf-KB pathways to synergy regulate inflammation.
