Objective To analyze the difference of the curative effect of early continuous lumbar drainage(LD)and conventional lumbar puncture(LP)in the treatment of aneurysmal subarachnoid hemorrhage after interventional embolization,and to explore whether it is a reliable method for the clinical treatment of such disease.Methods A retrospective analysis of 62 cases of a SAH patients in the Neurosurgery department of the First Affiliated Hospital of Dalian Medical University from September 2014 to September 2015,each of the patients after onset in 3 days after the embolization surgery.The data included age,gender,preoperative Hunt-Hess classification,Fisher classification,aneurysm location,and postoperative treatment.According to the method of postoperative treatment,the patients were divided into lumbar cistern drainage group(group LD)and lumbar puncture group(LP group).Compared the differences between the two groups of the several aspects in postoperative headache duration,the time of red blood cell<100×106/L in cerebrospinal fluid(CSF)and cerebrospinal fluid pressure below 180 mm H2 O,the rate of brain vasospasm,cerebral infarction,cerebral water occurrence after 3 weeks,and the rate of good statistical of Glasgow Outcome Scale(GOS).Data statistical methods: the count data using t test,measurement data and the rate of comparison using x~2 test,using SPSS19.0 statistical software for statistical analysis of data.Results All the patients in the LD group of 32 cases,30 cases of group LP.There was no significant difference in age,gender,preoperative Hunt-Hess classification,and the location of aneurysm between the two groups(P>0.05).In the duration of headacheand neck stiffness(t=6.042,P<0.05),the time of red blood cell<100×10?/L in cerebrospinal fluid(CSF)and cerebrospinal fluid pressure below 180 mm H?O LD group was significantly shorter time than the LP group.The incidence of brain vasospasm(x~2=4.4,P<0.05),cerebral infarction and hydrocephalus(x~2=7.02,P<0.05)were significantly lower in group LD than in group LP after 3 weeks.The rate of good statistical of GOS(x~2=4.728,P<0.05)of LD group was significantly higher than that of LP group after 3 weeks.Conclusion For patients with a SAH,after the interventional embolization of aneurysms,the early use of continuous lumbar cistern drainage can rapidly improve the symptoms of patients,reduce the incidence of complications,improve the prognosis of patients,it is worthy of clinical promotion.
