Introduction: When BSSO(bilateral sagittal split osteotomy)in bimaxillary surgery is performed for correction of the skeletal Class III malocclusion, there is a significant likelihood that a relapse will occur that will require orthodontic postoperative compensation to maintain satisfactory dental relationships.Orthodontic camouflage treatment is the one option to correct dental relationships, but accepting the skeletal discrepancy for obtaining occlusal stabilization. Methods: This report describes the treatment of a 29-year-old female with Class III incisor relationship on Class III skeletal pattern with unilateral posterior crossbite on left side. The patient underwent combined orthodontic and orthognathic treatment for correction of the skeletal Class III malocclusion 8 years ago.The treatment consisted of placement of a pre-adjusted MBT bracket system, interproximal reduction was performed on the mandibular incisors to compensate anterior teeth, and cross elastics were used to correct unilateral posterior crossbite. Results: The dentoskeletal and soft-tissue alterations occurred during orthodontic camouflage treatment of Class III malocclusion,by the superimposition method of Ricketts. These alterations included a bit backward mandibular movement, retraction of the mandibular incisors, and correction of the unilateral posterior crossbite. The result was satisfactory from both function and esthetic, providing adequate overjet and overbite. Conclusion: Orthodontic camouflage treatment might be a good option for patients with the Class III incisor relationship(edge-to-edge bite)and the Class III skeletal pattern who relapsed after orthognathic treatment.
