BackgroundGastric cancer is a kind of malignant tumor that occurs in gastric epithelial tissue,and it is one of the most common malignant tumors in the world.According to statistics,there were about one million new cases of gastric cancers worldwide,There were about 600 thousand people die from gastric cancer each year,which makes it the second in all cancer related deaths.Therefore early detection,early treatment and early intervention were important means to improve the survival of the patients.A large number of studies were made on the clinical features of gastric cancer of young and elderly patients,but there are few studies focusing on middle-aged patients.ObjectivesThe thesis is aimed at finding out the clinical features of gastric cancer in different gender and age groups,to analyze the differences among young,middle-aged,and elderly males,and that among young,middle-aged and elderly females,and to study middle-aged patients as an individual subject,so that more specific and comprehensive information can be obtained to the prevention and treatment of gastric cancer.MethodsBy going through and analyzing 263 cases of patients diagnosed with gastric cancer from January,2014 to January,2015 in Xuchang Hospital of Traditional Chinese Medicine,the thesis divided the patients,according to the age range of WHO in 2000 and genders,into young,middle-aged and elderly male groups and young,middle-aged and elderly female groups.Pathological and clinical differences in different gender and age groups were analyzed from an individual perspective and the data were processed with SPSS17.0.If there were distinct differences among different male groups,the multiple comparisons would be used to find out the reasons behind these differences.The same method would be also used in the female groups.Results 1.In this study,of the 263 cases of gastric cancers,186 were males and 77 were females and the male-to-female ratio is 2.41:1;eleven were young males and 16 were females with a male-to-female ration of 1:1.45;82 were middle-aged males and 26 middle-aged females with a male-to-female ration of 3.15:1;93 were elderly males and 35 elderly females with a male-to-female of 2.66:1.2.As to the lesion locations,this study finds out that the part that most easily get diseased of the cases could be put in such a sequence,gastric antrum > cardia fundus > gastric body > whole stomach.The incidence of gastric cancer in gastric body in elderly male group was higher than that in middle-aged males,and there was a significant difference between the two groups(P<0.05).There were no distinct differences in other parts of the stomach in the rest of the male groups and there were no significant differences in diseased parts of the stomach in female groups.3.Borrmann?among middle-aged male group was much higher than that among elderly male group(P<0.05).And there was no other distinct difference in this respect among all the male groups.No significant difference in regard of this classification has been detected among all the female groups.4.The study finds out that adenocarcinoma among elderly male patients was much higher than that in middle-aged and young male groups(P<0.05);mucinous adenocarcinoma among young male group was much higher than the other two(P<0.05);There were no other significant differences in terms of historical classification in the three male groups.Among the female patients,adenocarcinoma among young female patients was much higher than that among elderly female group(P<0.05),and there were no other significant differences in terms of historical classification in the three female groups.5.In the light of pathological classification,in this study TNM stage ? was much higher in young male group than in middle-aged male group(P<0.05),and no other significant differences in TNM stages have been detected in the other male groups.There was no significant difference in this respect among female groups.6.In terms of clinical manifestation the most common ones were abdominal pain,followed by inappetence,haematemesis,melena and some other symptoms.Abdominal pain was much more likely to happen among middle-aged male group than among young and elderly male groups(P<0.05).Marasmus occurs more frequently among elderly male group than middle-age male group(P<0.001).The incidence of dysphagia was also much higher in elderly male group than in middle aged male group(P<0.001).The rest male groups have shown no significant difference on various clinical manifestations.Incidence of marasmus in elderly female group was also higher than in middle-aged group(P<0.05),while the rest female groups have shown no significant difference on various clinical manifestations.7.There were no significant differences among different male and female groups in family,smoking and alcohol history.8.No significant differences among different male and female groups were detected in terms of patients who also suffer from high blood pressure and diabetes.Conclusions1.In this study incidence of gastric cancers among males was higher than that among females,but the number of females getting gastric cancers is increasing in the young groups in Xuchang district.2.This study detects that young patients of gastric cancers showed features of poor differentiation and high degree of deterioration,and female groups were of a higher degree of deterioration compared with male groups.Gastric cancers in middle-aged male groups were well differentiated,and more likely to suffer from abdominal pain.Marasmus occurs more frequently among elderly male group than middle-age male group,and incidence of dysphagia was also much higher in elderly male group compared with other age groups in Xuchang district.
