BackgroundAphasia is one of the common complications after stroke cerebral apoplexy.Motor aphasia which account for 20% of all aphasia,would make the patient avoid the normal social life and low the life quality because of speech disturbance with emotional disorder such as depression and impatient seriously to hinder the normal communication.In recent years,research shows that the treatment such as acupuncture,traditional Chinese medicine(TCM)and speech training method could improve the language function.But the recovery of language pathway has the characteristics of the long course and slow recovery.Therefore,it is important and significant to seek a more effective comprehensive treatment which would help patients to span the clinical-function as soon as possible,shorten the treatment schedule,make the patient to communicate with others in daily life,adapt to the needs of work and enhance patient confidence in the actual communication ObjectiveTo study Researching group therapy combined with speech training for improving effect of listening comprehension,oral expression ability,communication ability and the influence of the mood on motor aphasia after cerebral apoplexy.And provide a more effective treatment for speech rehabilitation training through combining traditional and practical comprehensive therapy model as to make patients communicate effectively with others,improve depressed mood,enhance the confidence in daily life MethodsMotor aphasia after cerebral apoplexy patients(n = 78),were randomly divided into the treatment group and the control group,39 patients in each group.The control group with uses the traditional "one-on-one" speech training,training topics include listening comprehension,oral English,reading comprehension and writing.It takes 30 min daily,six times a week,8 weeks as a course of treatment.The treatment group take with the open mode,work in groups of 3-5 on the basis of speech training group therapy,when there are much more patients during the same period,the second treatment team was established,group members including patients,families and speech therapist,assist members.Training topics mainly contain insinuation dialogues closing to daily life,role-playing,discussion topics,etc.It takes 1.5-2 hours,1 time a week,8 weeks as a course of treatment.All the patients data of communication ability,language function and depression would be analyzed by assessment personnel using SPSS19.0 statistical software before treatment,and after treatment of 4 weeks and 8 weeks.Results1.The result of Western Aphasia BatteryThere was no significant difference(P > 0.05)on two groups of patients with aphasia assessment score before treatment.Through 4 weeks treatment,there was significant improvement on the information volume,fluency,naming,repetition score for the treatment group rather than before;It is statistical significance(P < 0.05)on the amount of information,fluency and repetition score between the treatment group and control group.After 8 weeks treatment,It is statistically differences(P < 0.05)on the amount of information,fluency,listening comprehension,repetition,naming and retelling score between the treatment group and control group.Through 4 weeks treatment,there was significant improvement on the information volume,fluency,naming,retelling score for patients with high school education and it is statistically significant(P < 0.05)that patients with high school education had more improvement of the information volume,naming andreading scores than the others.After 8 weeks treatment,It is statistical significance(P <0.05)that patients with high school education had more improvement on information,listening comprehension,naming score than the others.That group therapy combined with speech training could improve patients with motor aphasia ability of reading listening,comprehension,oral expression,and the improvement of language function in patients with the high degree of culture is more obviously rather than the lowers.2.The result of practical communication ability:Evaluate of the practical communication skills in both groups before treatment had no statistical significance(P > 0.05).Through4 weeks treatment,there was statistically significant(P < 0.05)on improvement of practical communication skills by comparing with controls group.after 8 weeks treatment,there was significant difference(P < 0.01)on improvement of practical communication skills.It is statistically significant(P < 0.05)that patients with high school education had more improvement of practical communication skills than the lowers.3.The result of aphasia depression scale:There was no significant difference on aphasia depression score between the treatment group and control group patients before treatment.But through 8 weeks treatment,the treatment group had more significant improvement of the aphasia patients with depression scores than the control group on statistically significant(P < 0.05).The group treatment have significant improvement on depression.Conclusion1.Group therapy combined with speech training has significant improvement on the amount of information,listen to understand,retelling,naming and reading for patients in motor aphasia after cerebral apoplexy patients,and a high level of education can improve more significantly;2.Group therapy combined with speech training can help patients in motor aphasia after cerebral apoplexy improve practicability of language communication ability,promote the patient's language ability and the improvement of the communication strategy;3.Group therapy combined with speech training can help patients in motor aphasia after cerebral apoplexy overcome emotional disorders,reduce depression,enhance confidence into society.
