| Objective: To detection the differential expression of ANGPTL4 in lung cancer with malignant pleural effusion and tuberculous pleural effusion Methods:On the premise of obtaining informed consent,collected the clinical data in patients with malignant pleural effusion and tuberculosis pleural effusion in Wannan Medical College,the First Affiliated Hospital,Yi Ji Shan hospital from December 2014 to December 2015.By enzyme-linked immunosorbent assay(ELISA)method for the determination of the concentration of ANGPTL4 in the pleural effusion.Collect the medical records of CEA,CYFRA21-1,LDH and GLU in patients with pleural effusion.Applying EXCEL Electronic Form to Make Report Form of ANGPTL4,CEA,CYFRA21-1,LDH,GLU.The data were analyzed using SPSS 21.0 a statistical software(SPSS 21.0).After testing the sample data are non-normal distribution,so the experimental data between the two groups were compared using the Wilcoxon rank sum test,χ 2 test was applied to analyze the constituent ratio.Results:1、The concentration of Angptl4 in Lung cancer group were 26.03(20.23~44.36).In addition to LDH,the levels of Angptl4,GLU,CEA,CYFRA21-1 in Lung cancer group were higher than the tuberculosis group,the difference was statistically significant after the rank sum test.2、The concentrations of Angptl4,CEA,CYFRA21-1,LDH,and GLU were significantly different in lung cancer groups:in the adenocarcinoma group they were 32.46(23.64~76.67)、258.50(124.25~558.50)、44.85(31.64~81.41)、420.00(289.00~649.75)and 6.00(4.72~7.48)respectively;in the squamous cell carcinoma group,the concentrations of the five indicators were 86.54(61.59~119.01)、136.00(79.00~258.00)、81.20(53.12~94.27)、 450.00(253.00~889.00)、 6.50(5.47~7.31)respectively;in the small cell lung cancer group,the concentrations were 85.16(66.72~114.04)、264.50(109.25~587.75)、12.39(2.81~27.84)、498.00(408.00~740.05)、3.96(3.13~7.25)respectively.The rank sum test(α=0.05)was used to analyze the differences of concentrations,the group needed to be compared with the other two respectively.The concentrations of Angptl4 were only significantly different between the denocarcinoma group and squamous cell carcinoma group(P<0.05).Compared with small cell lung cancer group,CYFRA21-1 were both higher in the denocarcinoma and squamous cell carcinoma group(P<0.05),but,there’s no significant differences between denocarcinoma and squamous cell carcinoma group(P<0.05).There were no significant differences for the LDH、GLU and CEA in the three groups mutually(P<0.05).3、χ~2 test was used in sex ratio analysis in different pathological types,χ~2=14.276,P=0.001,according to α=0.05 level,the difference was statistically significant.4、In the Lung cancer group,the concentration of ANGPTL4 was 32.61(18.90~95.90)in less than 50 years old population.In the age great than or equal to 50 year olds was 68.57(26.55~90.90),P=0.434>0.05.According to the level of α= 0.05,the difference was not statistically significant;the concentration of ANGPTL4 was 72.11(52.84~101.75)in male.In female was 29.50(23.63~85.30),P=0.041<0.05.According to the level of α= 0.05,the differences have statistical significance;the concentration of ANGPTL4 was 17.53(13.49~39.57)in pleural effusion < 1/3 of the chest of patients in the concentration in patients with pleural effusion is more than or equal to 1/3 of the chest was 70.58(28.04~101.75),P=0.013<0.05.According to the level ofα= 0.05,the difference has statistical significance.Conclusion:1、The concentration of ANGPTL4 in malignant pleural effusionis higher than the tuberculosis group,suggesting that ANGPTL4 may play an important role in the diagnosis of pleural effusion;2、In different pathological types of lung cancer pleural effusion of ANGPTL4 in addition to adenocarcinoma and squamous cell carcinoma group,the differences were not statistically significant,suggesting that ANGPTL4 protein secretion is different in adenocarcinoma and squamous cell carcinoma by the influence of some factors,and it is worth further study;3、The sex composition of different pathological types of lung cancer were not all the same,and ANGPTL4 expression might be associated with the gender,pleural effusion volume(P < 0.05),indicating that the gender factors may have its unique role In the development of malignant pleural effusion,and has a certain relationship with the clinical manifestations(caused by pleural effusion);4、Combined detection of Angptl4,CEA,CYFRA21-1 and GLU can improve the diagnostic rate of malignant pleural effusion. |
