Objective: To study the preoperative diagnostic value of EUS when performed after CT or MRI in patients suspected of periampullary cancer.Methods: Clinical data of 72 patients with periampullary cancer who underwent conventional imaging scan(CT or MRI),EUS,exploratory laparotomy from January 2012 to December 2014 in hospital of Yi Jishan,divided in two group by whether tumor was detected by conventional imaging diagnosis,then calculate tumor mass detection and tumor accuracy of resectability of prediction.Results: A mass was visible on conventional imaging scan in 60/72(83%)patients.Among these 60 patients,EUS detected a mass in 57/60(95%)patients.In patients who without a visible tumor mass on conventional imaging scan,EUS detected a mass in 10/12(83%)patients.In patients who with a visible tumor mass on conventional imaging scan,resectability was accurately predicted by conventional imaging scan and EUS in 51/60(85%)and 52/60(87%),respectively(P=1.0).In patients who without a visible tumor mass on conventional imaging scan,resectability was accurately predicted by conventional imaging scan and EUS in 9/12(67%)and 12/12(100%),respectively(P=0.217).Conclusion: In patients,who suspected for periampullary cancer,with a visible mass on conventional imaging scan,EUS does not improve the accuracy of tumor resectability of prediction,should not be performed routinely.In patients without a visible mass on conventional imaging scan,EUS confirm the presence of a tumor.
