Objective: To observe and compare the incidence of postoperative complications of patients with breast cancer who received preoperative chemotherapy(NAC group) and direct operation(non-NAC group). To discuss the effect of preoperative chemotherapy on postoperative complications of patients with breast cancer.Methods: Sixty-six women with breast cancer who underwent mastectomy and axillary dissection were observed. All of them were treated at the Breast Center of the fourth hospital of Hebei Medical University from April 2015 to October. Thirty-two patients were included in the preoperative chemotherapy group, and thirty-four patients in the direct surgery group. The general characteristics of the two groups were evaluated before operation, including age, body mass index(BMI), liver function, hypertension, diabetes and other complications. Postoperative drainage and complications were observed, and the two groups were compared.Results: There was no statistical difference between the two groups in general characteristics, such as the average age, BMI and basic diseases, etc..The total incidence rate of postoperative complications had no significant difference between NAC group and non-NAC group(25.00% and 23.53%, ?~2=0.19, P=0.89). The average operation time of the two groups was 86.88±16.40 min and 85.00±15.08 min, the difference was not statistically significant(t=-0.57,P=0.57). The total drainage fluid in NAC group was significantly less than that in non-NAC group, 383.13±279.37 ml in NAC group and 531.71±376.92 ml in non-NAC group(t=-2.15, P=0.03). The average extubation time was 6.75±2.94 days and 7.41±4.00 days in the two groups, there was no significant difference(t=-0.74,P=0.46). There were 8 cases of seroma after operation, including 4 cases(12.50%) in NAC group and 4 cases in non-NAC group(11.76%)(?~2=0.01,P=1.00). There were 2 cases of skin flap necrosis after operation, including 2 cases(6.25%) in NAC group and 0 case in non-NAC group(?~2=2.19,P=0.23). Incision infection occurred in 4 patients, 1 case(3.13%) in NAC group and 3 cases(8.82%) in non-NAC group(?~2=0.94,P=0.61). The results suggested that there was no difference in the incidence of postoperative seroma, skin flap necrosis and incision infection between the NAC group and the non-NAC group. The incidence of postoperative systemic complications of the two groups were 3.13% and 2.94%, respectively, with no statistical difference(?~2=0.00,P=1.00).Conclusions:1 Preoperative chemotherapy can reduce the total drainage fluid of patients with breast cancer after operation.2 Preoperative chemotherapy does not prolong the postoperative extubation time of patients with breast cancer.3 Preoperative chemotherapy does not increase the risk of postoperative incision infection in patients with breast cancer.
