Objective:We will investigate the concomitant disease and complication in patients with RA, in order to provide more clinical characteristics for the diagnosis and treatment in RA.Methods:1. Retrospective investigation in 157 cases with hospitalized RA and were conducted. The clinical and laboratory data of patients were recorded in detail. And the influence factors associated with concomitant disease and complications were analyzed.2.SPSS19.0 statistical software was used for data analysis. For quantitative data meeting normal distribution or not, we used x±s and M (Q25, Q75) to describe data characteristics respectively. The quantitative data were compared by using t test or rank test; incidence data were used with the ?2 test. Logistic regression analysis was used to explore the factors affecting the comorbidity.Results:1.157 RA cases included in the analysis,122 cases were female,35 cases were male. The average age was 57±12.61 years, and average duration was 8(3,15) years. The prevalence of concomitant disease and complication was 84.71%.2. The most popular concomitant disease was cardiovascular system diseases (82 cases,52.23%), including hypertention (57 cases,36.31%), coronary artery disease (18 cases,11.46) and arrhythmia (7 cases,45%). musculoskeletal system (58 cases, 36.94%), including osteoarthritis (29 cases,18.47%), osteoporosis (56 cases,35.67%), femoral head necrosis (4 cases,2.55%); gastrointestinal system (14 cases,0.19%), including gastritis (10 cases,6.37%) and peptic ulcer (4 cases,2.55%); interstitial pneumonia (5 cases,3.15%), pulmonary infection (5 cases,3.15%), upper respiratory tract infection (3 cases,1.91%), abnormal cholesterol (46 cases,29.30%), hyperglycemia (19 cases,12.10%). Complications included increased platelet (57 cases,36.31%), anemia (51 cases,32.48%), hypoproteinemia (23 cases,14.65%). Respiratory tract infection happened in 8 cases (5.09%), high blood lipids in 46 cases (29.30%), high blood glucose in 19 cases (12.10%), hypoproteinemia in 23 cases (14.65%) etc.. Complications were mainly increased in 57 cases (36.31%),51 cases (32.48%) and so on.3. The incidences of hyperlipidemia and the increase of platelets in patients between 41 to 60 years old were the highest (P<0.05); and the incidence of anemia and osteoporosis in patients with long duration of RA was significantly higher (P<0.05). The incidence rate of hypertension was significantly higher in man than that in female (p<0.01). Risk factors related to hypertention were age, weight and total cholesterol; related factors to coronary artery disease were age, weight. risk factor to osteoporosis was age.Conclusions:Concomitant disease and complications of patients with RA is as high as 84.71%. The most common complication of the disease is cardiovascular disease, followed by hyperlipidemia, osteoarthritis, osteoporosis, digestive system disease, respiratory tract infection, etc.; and the complications of platelet increase and anemia are the most popular. At the active stage of the disease, the increase of blood platelet, anemia and hypoproteinemia were complicated; infection and pulmonary interstitial lesions were common in the respiratory system. Male, old age, long course of disease, high BMI are the risk factors for cardiovascular disease in RA.
