Clinical Research On The Effect Of Compound Danhong Injection On The Cognitive Behaviors Of The Patients With Vascular Cognitive Impairment

Objective:The research aims at observing the clinical efficacy of Compound Danhong injection on vascular cognitive impairment(VCI) patients.Methods : Sixty VCI patients meeting the given criteria were selected from the inpatients and outpatients of our hospital from January 2015 to December 2015. They were divided by computer randomly into Traditional Chinese Medicine(TCM)Treatment Group(30 patients) and Western Medicine Control Group(30 patients). The former group was injected with Compound Danhong injection while the latter donepezil by oral. The basic treatment of the two groups included anti-platelet aggregation, plaque stabilization, symptomatic treatment, etc. The research adopted randomization,comparison and single blind experimental methods, observing the changes of ADAS-Cog, MMSE, Mo CA, NIHSS and ADL scores and the TCM symptoms scores of the patients before and after the treatment.Results:(1) Cognitive function: After the treatment, the ADAS-Cog scores of the two groups decreased compared to those before the treatment, and there was an increase in MMSE and Mo CA scores. The abovementioned differences were statistically significant(P?0.01), but the score differences of the two groups after the treatment were not statistically significant(P ? 0.05). By comparing the sub-items' scores of the ADAS-Cog, MMSE and Mo CA of the Treatment Group before and after the treatment,it was noted that the scores of the sub-items of ADAS-Cog including words recalling,naming, ordering, words identifying, memory test ordering, difficulty in searching words, linguistic comprehension abilities, attention, etc went down dramatically(P<0.01); the score of intentional exercises dropped at a certain amount(P<0.05) and there was only a slight decrease of scores in terms of structural exercises, orientation and spoken abilities(P>0.05). When coming to the sub-items of MMSE, the Treatment Group showed significant improvement in memorizing ability, attention, calculating and recalling abilities. The difference was statistically significant(P<0.01); however, no big enhancement was seen in the orientation and language abilities. The difference did not have statistical significance(P>0.05). Regarding the sub-items of Mo CA, the Treatment Group patients improved impressively in attention, recalling delay with statistical significance of difference(P<0.01); there was certain improvement in naming ability(P<0.05)while no particular sign of enhancement in terms of visual and dimensional abilities, execution, language, abstraction and orientation(P>0.05).(2) Neurologic impairment and daily living ability: The scores of NIHSS and ADL of the Treatment Group before and after the treatment dropped obviously with statistical significance(P?0.01). For the Control Group, the NIHSS scores decreased slightly(P?0.05) while ADL fell at a certain amount(P?0.01).(3) TCM symptoms scores: After the treatment, the TCM symptoms scores of the Treatment Group dropped greatly(P?0.01) while those of the Control Group were not statistically significant(P?0.05).Conclusions:(1) Both Compound Danhong injection and donepezil can improve the cognitive ability of VCI patients and delay the course of disease. Furthermore, the improvement Compound Danhong injection brought mainly lay in memorizing ability, attention and recalling ability;(2) Compound Danhong injection functioned better than donepezil in easing neurologic impairment of VCI patients; but they took quite the same effect in improving the daily living ability;(3) Compound Danhong injection had a better effect than donepezil in terms of the TCM symptoms of the VCI patients;(4) Compound Danhong injection, safe and without untoward effects, achieved a satisfactory result for the clinical purpose.
