ObjectiveTo explore the clinical curative effect of posterior unilateral transpedicular debridement,decompression with interbody bone fusion and internal fixation for treatment of thoracolumbar tuberculosis.MethodsFrom January 2010 to January 2014, We treated 174 cases of thoracolumbar tuberculosis patients,97 patients accord with the inclusion criteria and Were included in this study,A group(the treatment group,53 patients) underwent posterior unilateral transpedicular debridement, bone fusion and internal fixation Surgery,B group(the control group,44 patients) underwent traditional posterior operation. compare and analysis the two groups To explore the clinical curative effect of posterior unilateral transpedicular debridement, bone fusion and internal fixation for treatment of thoracolumbar tuberculosis.ResultsAll of the patients through perioperative safely and were followed up for 12~48months, clinical symptom remission satisfied after operation,ESR and CRP decreased to normal range, All patients obtained interbody fusion at 3-9months,and no significant Kyphosis angle lost;All patients had obtained improvement of neurological function by 0-2 Frankel grades;. Compare A and B groups: the operation time, Blood loss, hospital stay after operation, Kyphosis angle lost and VAS score have no significant difference(p>0.05),the drainage of A group less than B group(p<0.05),the ODI score in group A after operation 3 months was less than group B(p<0.05) but have no difference in last follow-up; pre-operative and post-operative,the Frankel grade in two groups were not significantly different.1 caes in A group occurred psoas abscess one month after the operation and suffered second debridement operation get cured,there was no complications such as breaking or loossening of the screwsor rods.ConclusionsThe posterior unilateral transpedicular debridement, bone fusion and internal fixation for treatment of thoracolumbar tuberculosis show satisfied clinical curative effect, less invasive,stronger Spinal stability and more quickly Post-operative recovery,compare with traditional posterior Surgery.
