The Complication And Long-term Outcomes Of Laparoscopic Versus Open Surgery For Colorectal Cancer. A Meta-analysis

Objective: to compare the incidence of postoperative complications and the risk of long-term survival between laparoscopic abdominal surgery and Laparotomy colorectal cancer treatment differences.Methods: We searched published on laparoscopic and open treatment of colorectal cancer in randomized controlled trial(RCT) papers from 1998 to July 2015. extracted data for meta-analysis.Results: A total of 21 RCT literature included in the analysis. Postoperative infection of laparoscopic radical group(RR=0.77,95%CI: 0.62-0.96, P=0.02) incidence is lower than open surgery,postoperative ileus(RR=0.71, 95%CI: 0.54-0.92, P=0.01) lower incidence than the laparotomy group. postoperative bleeding(RR=1.21, 95%CI: 0.75-1.93, P=0.43), the incidence of urinary tract infections(RR=1.01, 95%CI: 0.71-1.44, P=0.95), voiding dysfunction incidence(RR=0.96, 95%CI: 0.57-1.61, P=0.87), two surgical postoperative pulmonary infection rate(RR=0.90, 95%CI: 0.64-1.29, P=0.58), anastomotic leak occurred rate(RR=1.11,95%CI: 0.84-1.74, P=0.47), the incidence of postoperative abdominal abscess formation(RR=1.07, 95%CI: 0.72-1.60, P=0.73) no statistical difference. Laparoscopic group overall survival risk(HR=0.85, 95%CI: 0.75-0.95) lower than the laparotomy group, the risk of disease-free survival(HR=0.93, 95%CI: 0.82-1.03) two groups was not statistically difference.Conclusions: Laparoscopic resection of colorectal cancer does not increase postoperative complications, disease-free survival with the same open surgery, the risk of long-term overall survival is less than open surgery The procedure is safe and feasible should be widely applied.
