Objective Follow-up by implementation of debridement therapy in patients with spinal tuberculosis,comparative study thorough debridement or not impact on the efficacy of spinal tuberculosis.Methods Patients diagnosed with spinal tuberculosis and 206 cases of surgical indications, male 93 cases,female 113 cases;mean follow-up time was 40 ± 2.8 months, including five cases of cervical,thoracic 63 cases, 15 cases of thoracolumbar, lumbar spine in 93 cases, 30 cases of lumbosacral. All patients before and after surgery routine SHRZ 2-8 weeks of TB treatment program, are used anterior debridement, interbody vertebra strut graft, anterior or posterior deformity correction, internal fixation. After 2 weeks of conventional X-ray, CT, MRI evaluation, in accordance with its accurate imaging patients divided into: A group(thorough debridement group of 154 cases)and B group(conventional debridement group of 52 cases). According to follow-up time, observe the projects,etc. patients were followed up to observe its effect.Results From preoperative and postoperative clinical manifestations, imaging findings, test results, living and working conditions of the two surgical options were evaluated, and to reflect postoperative recovery. No significant difference between the two groups before treatment; But there is statistically significant difference was observed before and after the above-mentioned indicators inherent treatment group comparisons. Existence thorough debridement group cure rate with conventional debridement group was statistically significant(P <0.05), thorough debridement group of high cure rate( 88.96%); thorough group( all patients) than the conventional group( all patients)anti-tuberculosis significantly shorter, there are statistically significant differences(P <0.05). Thorough group of more traditional group exists significant difference(P <0.05) in the preoperative and postoperative ESR and CRP.Conclusions On the normative premise of chemotherapy, thorough group of more traditional group was significantly lower recurrence rate in spinal tuberculosis. Traditional debridement only cleared the pus within the lesion, sequestrum, granulation tissue and caseous necrosis disc, resulting in lesions of spinal tuberculosis is not completely clear again or multiple surgeries important reason; and thorough debridement at its base clear the hardened walls, voids, bone lesions bridge, complete and thorough debridement, significantly reduced the relapse rate, improve the cure rate significantly shorten the course, thus providing a strong basis for further clarified spinal surgery program.
