Objective:1. Exploring the value that investigation of the brain functional connectivity in the brain functional changes in T2 DM.2. Investigating the relationship between the brain function connection changes and anxiety, depression, for diabetic patients with anxiety and depression of the early discovery and timely clinical intervention.3. Analyzing the clinical parameters and brain function changes, in order to explore the relationship between them.Methods:1. Subjects: They were 21 middle-aged T2 DM patients and 21 normal controls that were recruited from the People's Hospital of Henan Province and examined by a endocrinologist based on the 1999 version of guideline criteria of WHO.2. Data acquisition: Germany's Siemens 3.0 T MRI scanner should be used in all subjects' corresponding scanning, using the standard 8-channel coil type. The main sequence adopted by scanning is the conventional sequence axial T1 weighted images(TR/TE,220/2.61ms), axial T2 weighted images(TR/TE,3500/113ms), axial T2 weighted fluid attenuated inversion recovery(TR/TE,8000/96ms), sagittal view T1 weighted images(TR=220ms,TE=2.48ms). Layer thickness of 6.0 mm and inter-slice spacing 1.2 mm.3D T1 scan: Adopting the T1-FSPGR(TR=2300ms,TE =2.98ms) sequence to collect, TR=2300ms,TE=2.98 ms, layer thickness 1.2 mm, layer number 160.BOLD-fMRI scan: All subjects were asked to close their eyes, but keep a clear head, try to keep relaxed, avoid activities specific ideas, and in the process of scanning are required to keep don't move your head in this scan, the subjects and the corresponding soft mat is placed in the inside of the coil in order to avoid the appearance of motion artifacts. At the end of the scanning, inquiring all subjects to determine they had been operating according to the requirement in the process, those who violated the requirement of image will be out. Scan parameters mainly use gradient echo planar imaging sequences of acquisition, TE =30 ms, TR=2000, layer spacing of 0.5 mm, 4.0 mm thick, matrix of the 64x64, FOV=240x240 mm, layer number 33.3. Image post-processing: This research mainly using REST packages and SPM5(statistical parametric mapping), and on the Matlab7.0 platform for the corresponding data analysis and processing. In the process of analysis, mainly by the AAL template as the foundation, the PCC/hippocampus as seed regions, corresponding to return to the area after the seeds of blood oxygen level dependent signal time series is extracted, and its whole brain and other areas based on voxel correlation analysis of time series, to gain the significant connection areas. The last, adopting the two sample t-test to obtain the function of the case group and the control group connection. In the study of multiple comparison mainly adopts AFNI provide AlphaSim procedures.4.Statistical Data analysis: This study adopted the appropriate statistical methods, the research firstly got the corresponding f test on the data of the case group T2 DM and control group, after by using Student t test for clinical measurement data for the corresponding statistical analysis. And this study then using the chi-square test for the gender between case group T2 DM and control group to finish the corresponding statistical analysis. Using independent sample t test for the score of SAS, SDS MMSE between case group T2 DM and the control group for the corresponding statistical analysis. All the study of the associated with magnetic resonance(NMR) statistical analysis using FSL4.1 has carried on the corresponding processing, at the same time, other statistical analysis are adopted spssl6.0 software(SPSS, Chicago, IL). Study about clinical information and cognitive aspects of the data using the mean(+ /-standard deviation) to say, at the same time, P < 0.05 was considered statistically significant.ResultsAfter the buckle belt back as the seed regions, used as basis for the function of connection method find that the case group and the control group after the button back and anterior cingulate back function connection strength decreased significantly; SAS scores, SDS scores HbAlc and BMI were inversely related to the buckle back and anterior cingulate back function connection.ConclusionCompared with normal subjects. Type two diabetic patients of brain functional connectivity changes. To provide an effective method to study the brain function in type two diabetic patients. And the relation the changes to emotional, perhaps it will develop a new early detection method for patients with anxiety or depression that enables clinicians to early detection of risk factors, early intervention for diabetic patients. Connecting the brain function of patients with type 2 diabetes and HbAlc were negative correlation, indicating that in the course of clinical treatment, it is very important to closely inspect and control the HbAlc.
