ObjectiveTo discuss the features of the4heart chambers diameter?left ventricular ejectionfraction?serum concentration of BNP in elderly chronic heart failure paitents.Materials and methods1.GroupsFrom June1st2013to December31th2013,259paitents were selected into this trialin Geriatric Department in a hospital in Taian city.Patients with cardiac function grade(NYHA) II-IV were recruited into subgroup I,heart failure group.Paitens with cardiacfunction grade(NYHA) I,without symptoms of cardiac disfounction were recruited intosubgroup II,non-heart failure group.In subgroup I, a total of137paitents were selected,among them80were men and57were women. Age range:49-96,average age:74.5±9.0.Clinical symptoms: dyspneaafter minor activity,nocturnal paroxysmal dyspnea, orthopnea,body edema,fatigue, ane-pithymia, decreased urine output, etc.Basic diseases:coronary heart disease, hypertension,diabetes, etc.Patients with comorbidities adult congenital heart disease(ACHD), rheumaticheart valve disease? dilated cardiomyopathy (DCM),hypertrophic cardiomyopathy(HCM),infectious endocarditis were excluded.In subgroup II, a total of122paitents were selected, among them58were men and64were women. Age range:45-94,average age:72.9±11.4.No symptoms of cardiac disfounc-tion. Exclusion criteria was same as group I. There was no statistical difference in basicdiseases between these two groups.From group I,58paitents were selected into another subgroup,among them32weremen and26were women,.Age range:48-92,average age:73.6±10.0.2.Methods2.1Condition assessment of all patients after admission, measuring blood pressure, heart rate, and for regular check, such as the three routine, liver and kidney function, bloodlipids and sugar, statistical diagnosis;2.2All the paitents were arranged to one echocardiograpic test,and measure aortic AO,right ventricular outflow tract RVOT, interventricular septum thickness IVSD?AAO, rightventricular RV, left the aortic root, interior diameter LVD, right or left atrial LA RA, leftventricular end-diastolic volume LVPWD, pulmonary artery, PA, left pulmonary arteryLPA, RPA right pulmonary artery, left ventricular ejection fraction LVEF, pulmonary arterypressure.2.3Group III uederwent BNP test.3.Statistical analysis methodUsing SPSS17.0software packages to analyze the test data.3.1The comparison of echocardiographic index between diffrient age groups in groupI by F test, p values <0.05is significant;3.2The comparison of echocardiographic index between diffrient age groups in groupII by F test, p values <0.05is significant;3.3The comparison of echocardiographic index between the two groups patients olderthan60by t test, p values <0.05is significant;3.4The comparison of serum concentration of BNP between diffrient age groups ingroup III by F test, p values <0.05is significant;3.5Comparison between different gender about the serum level of BNP in patientsgroup HF by t test, p values <0.05is significant;3.6Correlation analysis among serum BNP concentration?b.p.m and BP,by Pearsoncorrelation analysis, r?0.7, are highly correlated;0.4?r <0.7, moderately related;0.2?r<0.4, low correlation; r <0.2, weak relevant.Result1. Heart failure patients older than60years old compared with the same age patientswithout heart failure,left atrial LA larger (p=0.004), Left ventricular diameterbroaden(p=0.055),left ventricular ejection fraction LVEF decreased (p=0.000);2. The comparison of echocardiographic index between diffrient age groups in HFgroup, p values?0.05, means no significant differences;3. The comparison of echocardiographic index between diffrient age groups in no HFgroup, p values?0.05, means no significant differences;4. Concentration of serum BNP elevated in heart failure patients older than60years old,?100pg/ml, no significant differences between different age groups and diffrientgender;5. Correlation analysis among serum BNP concentration?b.p.m and BP, r <0.2, weakrelevant.Conclusion1. Heart failure patients older than60years old compared with the same age patientswithout heart failure,cardiac remold mainly for the left atrial LA larger (p=0.004), Leftventricular diameter broaden,left ventricular ejection fraction LVEF decreased (p=0.001);But the LVEF of the two groups both?50%, considering heart failure in elderly are mostlydiastolic heart failure (DHF).2.Age does not leads to cardiac remodeling.3.Elderly heart failure patients, concentration of serum BNP higher than the normalstandard, but age?gender?BP does not affect the level of serum BNP.
