As the most convenient and effective medical treatment, medication is the most important intervention of health services. However, irrational drug use currently causes tremendous harm to the patients. In America, the death caused by medication is more than 100 thousands. The situation is more serious in China. The reason lies in the medical negligence during the clinical practices, as well as the explosive growth of medical knowledge which exceeds the ability of the clinicians to acquire. Therefore, some new informatics methods, such as medication relative decision support system, have been introduced into the workflow of clinical prescription to reduce the iatrogenic injuries. Although the effectiveness of these systems for the medication security has been confirmed in some clinical studies, some problems are found, such as the insufficiently personalized medication rules, many invalid warnings caused by average rules and consequent warning fatigue. Therefore more clinical pharmacist has been introduced to assist the clinicians to perform more reasonable selection, use and estimation of medication.To assist clinical pharmacist solving the knowledge management and pharmacy surveillance informationized problem, knowledge base driven clinical pharmacy surveillance system is put forward. Firstly is the study of the system construction, of which the knowledge base and the framework building of clinical pharmacy service are the central contents. Then the concrete contents of the pharmacy knowledge base are elaborated. Finally introduces the clinical pharmacy services of the system. The surveillance system offers the routine pharmacy service, rational drug use support, drug consumption statistics and pharmacy surveillance of the important clinical process.In this paper, we assess the system with a case practice. Result shows the system is able to help the clinical pharmacist increase efficiency and they appreciate our job.
