Objective To determine the efficacy of long time noninvasive positive pressure ventilation(NPPV) in the late period of stable chronic obstructive pulmonary disease(COPD).Methods Trials were located through electronic searches of PubMed、EMbase、CNKI、WanFang、Wei Pu、CBM(from January 1990 to May 2015). Statistical analysis was performed with The Cochrane Collaboration’s software RevMan 5.3.Results 24 trials contains 1280 patients were included. Results showed that NPPV can Reduce PaC02[OR =-13.11, 95 % CI(-15.97,-10.26),P<0.00001],improve Pa02 [OR=9.99,95%CI(7.91,12.06),P<0.00001];Two years of NPPV Can delay the decline of Lung function;Increase 6MWD in patients with stable COPD[OR=66.02,95%CI(34.11,97.94),P<0.0001];Reduce mortality[OR 0.62, 95% CI(0.47 to0.82),P=0.0006].Conclusion 1).NPPV Can Improve patients with arterial blood gas, and the application of NPPV the longer, the more obvious effect.2).NPPV Can Increase patients exercise endurance, and improve the quality of life.3).Long application of NPPV can delay the decline in FEV1 and FVC(Two years or longer) 4).NPPV Can reduce the patient’s mortality.
