The Curative Effect Analysis Of High Intensity Focused Ultrasound Combine With TAE And PEI To Treat The Primary Hepatic Carcinoma

Objective:According to the study of HIFU combined with TAE and PEI to treat the PHC,and analyze the factor of the ablation efficiency.So that we can choose an individualized treatment plan for patients,not only we improve remarkable curative effect, and can reduce the occurrence of complications.Method:The 40 patients accord with into the set of conditions can be divided into A, B two groups.A group:HIFU treatment before the first treatment within 1-2 TAE.B group:HIFU treatment half an hour before PEI.Observed:The change in symptoms before and after treatment,the ALT、 AST 、 STB 、 AFP and imaging situation after one week and three months.Results:1. Improvement of symptoms:Group A(HIFU + TAE) 15 patients(83.3%); Group B(HIFU + PEI) 14 patients(93.3%). P>0.05.2. Laboratory tests:Before treatment:Group A ALT 60.4±65.8U/L,AST 74.4±83.6U/L and STB 19.2±11.0umol/L; Group B ALT48.1±44.1U/L,AST 47.9±26.2U/L and STB 20.2±23.7umol/L.One week after treatment:Two groups ALT are higher,80.75±96.6U/L and68.8±58.4U/L respectively, P < 0.05.Two groups AST has no changed,75±81.2U/L and 50.3±18.6U/L respectively, P > 0.05.Three months after treatment,two sets of ALT and AST compared with before has declined.ALT were 33.2±15.7U/L and 38.6±21.7U/L respectively.AST were 46.9±35.0U/L and 39.5±37.1U/L respectively,P<0.05.Two groups of patients before and after treatment STB and APF were little changed, after statistical analysis, P> 0.05.3. Ablation efficiency: the total effective rates of two groups were65% and 30% respectively, progression rates were 25% and 45%respectively.Conclusion:1.Combination therapy of HIFU is optional, safe and has no serious side effects for primary hepatic carcinoma.2.After a week of combination therapy of HIFU, the liver function is lessening. Three months after treatment, liver function returned to normal.And it had no significant effect on STB and AFP.3.TAE and PEI have a cooperative effect to HIFU. HIFU combinedwith TAE is better than HIFU combined with PEI for PHC of ablation therapy and prognosis. The blood of tumor is an important factor affecting the efficiency of ultrasound ablation.
