Objective: To compare the clinical value of transvaginal color sonography(TVS) and magnetic resonance imaging(MRI) in the diagnosis of deep infiltrating endometriosis(DIE). Method:118 cases of DIE highly suspected before surgery were collected in our hospital from January 2013 to January 2016 and all undertaken transvaginal sonography(TVS) and magnetic resonance imaging(MRI). As surgery and histology being the final diagnosis standard, the diagnostic value of transvaginal sonography and magnetic resonance imaging was compared. Results: 1.The accuracy, positive likely ratio, Kappa confidence of TVS/MRI were, respectively, 90.68%/92.37%、10.86/10.17、0.61/0.67 for all pelvic DIE patients.2. The accuracy, positive likely ratio, Kappa confidence of TVS/MRI were, respectively, 87.3%/86.44%、11.75/4.75、0.69/0.64 for uterosacral ligament DIE.3. The accuracy, positive likely ratio, Kappa confidence of TVS/MRI were, respectively, 88.14%/86.44%、6.10/7.29、0.72/0.69 for rectouterine pouch DIE.4. The accuracy, positive likely ratio, Kappa confidence of TVS/MRI were, respectively, 83.05%/91.53%、7.67/6.80、0.69/0.83 for rectum DIE.5. The accuracy, positive likely ratio, Kappa confidence of TVS/MRI were, respectively, 90.68%/88.14%、21.48/9.65、0.73/0.68 for rectovaginal spectrum DIE.6. The accuracy, negative likely ratio, Kappa confidence of TVS/MRI were, respectively, 94.07%/86.44%、0.41/0.2、0.71/0.56 for vaginal fornix DIE.7. The accuracy, negative likely ratio, Kappa confidence of TVS/MRI were, respectively, 95.76%/97.46%、0.5/0.3、0.65/0.81 for ureter DIE.8. The accuracy of TVS/MRI were, respectively, 90.68%/92.37% for bladder DIE. Conclusion: 1. TVS and MRI have high clinical value in the diagnosis of pelvic infiltrating endometriosis.2. The diagnostic value of TVS in the special pelvic structures is higher than that of MRI. And the diagnostic value of MRI in the important pelvic organs is higher than that of TVS.3. Due to the economic and good repeatability, TVS should be as the preferred examination, and MRI is performed in highly suspected DIE, especially in the important organs of the pelvic.
