Background and Objective:At present the research about the relation between IDH1 gene with AML has become a hot spot at home and abroad,but IDH1 gene mutations on the clinical characteristics and prognosis of patients with AML relationships is still controversial,such as no unified conclusion.This topic using Meta analysis method to evaluate IDH1 gene mutations in AML patients clinical characteristics and prognosis.Methods:The Collaboration system evaluation method in order to “acute myeloid leukemia,isocitrate dehydrogense IDH” key words as a search term,computer retrieva“MEDLINE”with“acute,myelogenous,leukemia,isocitrate,dehydrogenase”as a search term,computer retrieval CNKI,Wan Fang Date and weipu Date.For the above datebase,in November 2015 deabline for all relevant IDH1 gene mutations in acute myeloid leukemia,cohort study on the control of the literature.In peripheral blood leukocyte count,platelet count,hemoglobin count,proportion of primitive cells,bone marrow and associated with survival and disease-free survival as the evaluation index,using Rev Man5.2software for meta-analyses.Results:A total of 12 study literature,including5408 cases of subjects, including IDH1mutations(IDH1+)412cases of AML patients,no mutant IDH1 group(IDH1-) of 4996 patients with AML.Meta analysis showed :IDH1 gene positive in patients with acute myeloid leukemia patients with acute myeloid leukemia with IDH1 gene negative overall survival(OS) between the risk ratio(HR) of 1.45(95%CI:1.22-1.71,P<0.001);Disease-free survival(DFS) risk ratio(HR)of 1.73(95%CI:1.77-2.56,P=0.006),prompted IDH1+ theprognosis of patients with AML is abvious difference in the IDH1- patients,the difference was statistically significant,but relapse-free survival(RFS) in risk ratio(HR) of1.22(95%CI:0.79-1.87,P=0.38),the difference between IDH1+ with IDH1- group,there was no statistically significant difference.Hemoglobin in IDH1+ AML patients(Hg),platelet counts(PLT) the IDH1-group increased significantly,the effect of statistic MDand 95%CI respectively [MD=2.91,95%CI(0.26-5.57),P=0.03] and[MD=26.93,95%CI(14.08-39.78), P<0.001],while in the white blood cell count(WBC)and there was no difference in the two groups on primitive bone marrow cells(%),the effect of statistic MD and 95%CI respectively[MD=2.91,95%CI(-26.21-5.61),P=0.87]and[MD=4.87,95%CI(-3.83-13.57),P=0.27],there was no statistically significant difference.Conclusion:According to the results of the meta-analysis IDH1+ AML patients with IDH1- group from overall survival(OS),disease-free survival(DFS) compared with IDH1 gene is a risk factor.The prognosis of patients with AML group IDH1+ AML patients on clinical characteristics has high hemoglobin and platelet count,in proportion to the original white blood cell count and bone marrow cells on no difference. Due to the quality、quantity such as differences in literature,but also need to further research.
