| Objective:Elderly patients with T2 DM often concomitant with various diseases,such as cardiovascular diseases,respiratory diseases and cerebrovascular disease,which make the glucose control is different from other age groups.Through the observation of metabolism index including glycosylated hemoglobin,fasting blood glucose,postprandial blood sugar, age,waist circumference,body mass index,systolic blood pressure,diastolic blood pressure triglyceride,total cholesterol,high density lipoprotein-cholesterol,low density lipoprotein-cholesterol,hemoglobin,red blood cell count,mean corpuscular volume,acute and chronic complications of type 2 diabetes mellitus,cardiovascular diseases,respiratory system diseases,cerebrovascular disease,altitude.We can observe and analyze the level and influencing factors of glycosylated hemoglobin in elderly patients who suffering from type 2 diabetes mellitus,in order to provide treatment guidance for this special population.Methods:According to world health organization(WHO) diabetes diagnostic criteria of 1999,326 patients with type 2 diabetes mellitus(T2DM) and the age is more than 65 years with 146 male patients and 180 female patients from endocrinology department in Affiliated Hospital of Qinghai University from 2014.5 to 2015.5.All of the study subjects were measured body height,weight,waist circumference,systolic blood pressure(SBP),diastolic blood pressure(DBP)and body mass index(BMI) is calculated.All of the study subjects were collected fasting venous blood in roder to determineglycosylated hemoglobin(Hb A1c),fasting blood glucose(FBG),postprandial blood glucose,triglyceride,total cholesterol,high density lipoprotein-cholesterol,low density lipoprotein-cholesterol,hemoglobin(HGB),red blood cell count(RBC),mean corpuscular volume.we need to record age,duration of diabetes mellitus,altitude,acute and chronic complications of type 2 diabetes mellitus(T2DM),cardiovascular diseases, respiratory system diseases, cerebrovascular disease.All of the study subjects were divided into three groups according to the level of Hb A1 c.All data analysis were completed by SPSS19.0.Results:1.People account for 53.8% of the total number in the elderly patients with type 2 diabetes mellitus,people don’t have good control of their blood glucose.2.Three groups of age, WC, BMI, FBG, PBG, LDL-C, duration of T2 DM, acute and chronic complications of T2 DM,cardiovascular disease score, respiratory disease score, cerebrovascular disease score have significant difference(P < 0.05),three groups of SBP,DBP,TG,TCH, HDL-C,HGB,RBC,MCV,altitude have no significant difference(P > 0.05).3.Multiple linear stepwise regression analysis show that the Hb A1 c were linear correlated with age,BMI, LDL-C,duration of diabetes mellitus, acute and chronic complications of T2 DM,cardiovascular diseases, respiratory system diseases, cerebrovascular disease,FBG,PBG(P<0.05)Conclusion:People in this area don’t have good control of their blood glucose,most of the patients with high blood glucose levels,Hb A1 c were mainly influenced by age,BMI, LDL-C,duration of diabetes mellitus,acute and chronic complications of T2 DM,cardiovascular diseases,respiratory system diseases,cerebrovascular disease,FBG,PBG.Hb A1 c were increased with the increase of the above mentioned indexes,blood glucose is more difficult to control,On the contrary,people who with high blood glucose level are more susceptible to above diseases:WC, BP,TG,TCH,HDL-C,HGB,RBC,MCV,altitude has a very small influence on Hb A1 c. |
