BackgruondWarfarin is an anticoagulant widely used in treatment and prevention of deep vein thrombosis, pulmonary embolism, atrial fibrillation(AF) and other thromboembolic diseases. Due to its narrow therapeutic range and wide interpatient variability in warfarin, individualized dose adjustment is particularly prominent in its clinical application. The pharmacodynamic activity and dose requirements of warfarin is attributed to several factors, the most clear and fixed individual differences are related to genetic, which has important clinical value. Studies show a variety of related gene is highly related to the efficacy of warfarin, and genetic polymorphisms in these genes may modulate the anticoagulant activity of warfarin. CYP4F2 is one of these genes. A strong association was found between CYP4F2(rs2108622) V433 M and interindividual variability in the anticoagulant effect of warfarin. The relationship between CYP4F2(rs2108622)genetic polymorphism in different races and maintenance warfarin dosage were researched in this paper.AimsTo evaluate the impact of CYP4F2 genetic polymorphism on warfarin dosage requirement.MethodRelated databases were searched, the relationship between CYP4F2(rs2108622)V433M C-T genetic polymorphism and maintenance warfarin dosage were researched, Meta-analysis was performed using revman5.0 software,calculate the WMD in groups and 95% confidence interval,then obtain the forest maps and funnel plots.ResultTotal 17 articles( 3559 patients) were included in this meta-analysis.It includes 10 articles(2460 patients) in yellow race, and 10 articles(1099 patients) in other race. The CYP4F2 CC,CT and TT genotypes accounted for 44.98%,38.49% and 16.52%,respectively,of CYP4F2 genotype in all groups. Compared with CYP4F2 CC group, maintenance warfarin dosage in CT group, TT group and CT+TT groupincreased about 14%(95%CI 12~16,Z=14.20,P<0.05), 8%(95%CI 4~13,Z=3.49,P<0.05)and 9%(95%CI 3~16,Z=2.72,P<0.05).The maintenance warfarin dosage increased with increasing T allele. Conclusion: The CYP4F2 allele in yellow race frequencies were difference from other racial population,and there is difference in the requirement of warfarin dosage.
