| Objective:To confirm early noninvasive mechanical ventilation treatment of acute left heart failure have a significant effect and through the noninvasive heart function monitor,cardiac output is improved in patients with acute left heart failure,who were treated by noninvasive mechanical ventilation.Methods:According to the criteria for the selection from January 2013 to June 2014 in ICU ward, a total of 40 cases with acute left heart failure patients for the first diagnosis. All patients were treated by general therapy, etiology therapy and drug therapy of acute left heart failure treatment guidelines in 2010;Control group a total of 20 cases used the oxygen mask while the experimental group a total of 20 cases were treated by noninvasive ventilator, until patients appeared in the process of treatment in conformity with the exclusion criteria, averted noninvasive mechanical ventilation to invasive mechanical ventilation. Monitored the change such as symptoms, heart rate, blood pressure, blood oxygen, cadiac output,marked effective time and BNP after 12 h in the process of treatment. Analyzed the data.Results:(1)After 1h, control group 16 cases were effective, 10 cases had marked effect, the total effective rate was 80%;Experimental group 18 cases were markedly effective, 19 patients effectively, the total effective rate was 95%.There was 1 case(5%)endotracheal intubation in the the experimental group and 4 cases(20%) in control group. Two groups of endotracheal intubation rate is statistically significant(P <0.05).The average marked effective time of the two groups have statistically significant differences(P<0.05).(2)Symptoms of two groups of patients and the clinical observation index after treatment were improved to varying degrees.But the effect of the experimental group is much better.Experimental group compared with control group, with significant difference(P < 0.05).Visible noninvasive ventilation is effective in treatment of acute left heart failure.(3)In the two groups before treatment,the parameters,such as the heart rate, breathing rate, SP02, PO2/Fi O2,SBP, CO, has no statistical differences(P > 0.05). After1 hour,the heart rate, breathing frequency, SP02, PO2/Fi O2, CO of experimental group declined obviously and there was statistically significant in experimental group and control group(P < 0.05).(4)After 12 h,the BNP of the two groups had significant difference(P < 0.05). Conclusion: Early noninvasive mechanical ventilation treatment of acute left heart failure have a significant effect and cardiac output is improved in patients with cute left heart failure,who were treated by noninvasive mechanical ventilation.Therefore, noninvasive ventilation may be effective in the treatment of acute left heart failure. |
