| A hernia is defined as the protrusion of viscera from the normal position of the abdominal cavit y through a channel or defects. In clinic, artifial patches were abroadly used for the repairing of the defects which formed in patients abdominal wall. This research aimed to develop a novel decellulari zed matrix material made from bovine pericardium for hernia repair, which treated by decellularizat ion, fixed with special crosslink agent and irradiated with 60 Co. And then followed by a series of te sts, such as physical, chemical and biological properties. The main results were as follows: tensile st rength>1MPa, elongation at break>10%, suture strength>5N, bursting strength>5MPa, the cells rem oval efficiency of tissues achieved to 99%, DNA Content<100ng, and demonstrated no cytotoxicity.60 Co irradiation procedure was adapted to sterilize the acellular martrix samples under 25 KGy. Th e removal efficiency of indicator virus, such as vesicular Stomatitis Virus, infectious canine hepatiti s virus, coxsackie virus, were all more than 4logs.A total 24 New Zealand white rabbits were randomly divided into three groups. Abdominal wall defect was built in aniamal experimental model. Polypropylene mesh, acellular matrix patches from bovine pericardium or e-PTFE mesh were applied in each group respectively. Simultaneously, each group received an injection of 5 drops of staphylococcus aureus liquid into incision. Postoperative observation points were designed 1 month and 3 months. Wound infection and adhesion of abdominal cavity were checked and recorded respectively on schedule by bacterial culture and pathological section. The results showed that acellular matrix material had effective ability of resisting infection and preventing the viscera adhesion. Polypropylene mesh had the same capacity in tolerated infection, but it was weak to prevent the viscera adhesion. e-PTFE mesh prevented adhesion effectively, but it had a weak ability of tolerated infection. So, acellular matrix pateces were proved with good performance in hernioplasty.In clinic, the acellular matrix material which made from bovine pericardium were used in adolescent undergoing hernia surgery for 50 cases, after 12-24 months of follow-up, no missing visit, wound infection, henia, stab foramen hematoma, chronic pain, foreign body sensation or hernia recurrence were not found during the observation period. Another 1 Case of infrequent abdominal giant incisional hernia was treated with acellular matrix patch, operative incision was primary healing, the bowel remains functional, and it had no recurrence. Clinical trial revealed acellular matrix material for repairing incisi onal hernia of abdominal wall could effectively reduce the incidence rate of postoperative complications and recurrence, and improve the quality of life greatly. |
