Objective: To Summarize the endovascular treatment experience of paraclinoid micro aneurysms(diameter ≤3 mm), and investigate the related factors which influence the efficacy.Methods: The clinical data of 34 paraclinoid micro aneurysms in 31 patients who received endovascular therapy between June 2011 and October 2014 were reviewed retrospectively, including 9 cases of male,female 22 cases, 25 cases of ruptured aneurysm, 6 cases unruptured aneurysms, all patients were carried out under general anesthesia endovascular embolization. the result in the DSA imaging showed the degree of aneurysm embolism and unobstructed parental artery, patients who come to our department at 3 months after hospital discharge had a outpatient follow-up, to sure whether patients recovery and no residual neural dysfunction, patients who come to our department at 6 months after hospital discharge had a DSA follow-up, to observe the degree of aneurysm occlusion and sure whether recanalization of aneurysm, finally, to assess endovascular treatment long time efficacy.Results: All patients were successfully embolized, 28 patients(90.3%)received stent assisted soil embolization, while 3 patients(9.7%) received simple coil embolization. Angiography after embolization operation showed that 30 patients(96.8%) received complete embolization, while 1patient(3.2%) received subtotal embolization. All patients were followed-up by outpatient service or telephone for 3~38 months after discharge; 26 patients received outpatient follow-up 1 time, while 5patients get one telephone follow-up 1 time, 29 patients recovered well with no or mild neurological dysfunction, one patient was severely disabled, and 1 patient died; 25 cases of ruptured aneurysm patients, 23 cases recovered well and no or mild nerve dysfunction, 1 case of severe limb paralysis, 1 case died after hospital discharge. Six other patients with unruptured aneurysms were recovering well, with no or mild nerve dysfunction. 25 patients received follow-up digital subtraction angiography at 6~24 months post- discharge. Follow-up digital subtraction angiography(DSA) showed no recurrence of the aneurysm.Conclusion: Endovascular treatment of paraclinoid micro aneurysms is an effective method, the morbidity, mortality and recurrence rate is low,but larger sample research and long-term clinical follow-up are still needed.
