Objective To investigate the clinical efficacy and complication of hematopoietic stem cell transplantation for acute myeloid leukemia.Methods 40 patients received HSCT,14 patiens were treated with auto-HSCT and 26 patients with allo-HSCT. In 26 cases who accepted allo-HSCT from siblings,19 cases were HLA-identical(6/6), Three were 2 HLA-antigen mismatched(4/6),4 cases were 3 HLA-antigen mismatched(3/6).Results 38 of 40 patients reconstituted hematopoiesis,with the median time of graft survival of neutrophils and platelets being 12 d vs 14 d and 14 d vs 20 d in auto-HSCT and allo-HSCT groups. The incidences of HVOD,HC and CMV were 0%,25% and 10%,respectively. The incidence of Acute GVHD was 23.1% and of cGVHD was 30.8%.27 cases were disease-free survival from 1to 47 monthe.,and 6 cases were disease relapse.Conclusion HSCT is safe and an only effective approach for the cure of patients with AML. In the case of can’t find the donor, auto-HSCT can be used as a consolidation treatment after CR, achieve the goal of prolong patient life. Compared with auto-HSCT,allo-HSCT,the recurrence rate is low, the majority of patients can get clinical cure.For refractory AML, allo-HSCT is the only hope for cure. Finally,if there are no human leukocyte antigen-identical sibling donors,haploidentical HSCT is an effective and feasible treatment for patient with acute myeloid leukemia.
