Background:The goal of this study was to explore material selection in the surgical treatment of Posterior malleolar fracture and analyze the clinical therapeutic effect about the surgical treatment of posterior malleolaor fracture.Hope to provide some references for the design of clinical surgical treatment.Methods:Collecting the clinical data of 66 patients who underwent posterior malleolaor fracture fixation from September 2011 to September2015 in our hospital.27 patients(8 males and 19 female) in group A whose fracture size ≤25% of the distal articular surface treated by the Cannulated Lag Screws,aged from 19 to 66 years.An average age of 43.56 ± 12.61.30patients(8 males and 22 female) in group B whose fracture size ≥25% of the distal articular surface treated by the Cannulated Lag Screws,aged from21 to 81 years.An average age of 46.83 ± 16.27.9 patients(1 males and 8female) in group C whose fracture size ≥25% of the distal articular surface treated by the lock/pressure support plate,aged from 27 to 67 years.An average age of 45.33 ± 14.47.Outpatient follow-up after operation andevaluate the patients’ ankle function at the last follow-up with using AOFAS rating system.Compared the operation time,amount of intraoperative bleeding,postoperative bleeding,hospital stays,full load time and AOFAS score.Results:A total of 66 patients with posterior malleolaor fracture for an average of 25 months(range,6-53 months) follow-up, All the patients reached the fracture concrescence.There were no statistical differences between group A and group B in the operative time,postoperative bleeding and full load time(P>0.05).There were no statistical differences between group B and group C in all of the data(P>0.05).There were statistical differences between group A and group B in intraoperative bleeding,length of hospital stay and AOFAS score(P < 0.05).Conclusion : There is no difference between the Cannulated Lag Screws and lock/pressure support plate in the clinical therapeutic effect that Involve in fracture size ≥25% of the distal articular surface in the posterior malleolaor fracture.Both of the fixations are effective.The Cannulated Lag Screws has the advantages of less intraoperative bleeding,length of hospital stay and AOFAS score in the treatment of posterior malleolaor fracture which involve in fracture size ≤25% of the distal articular surface compared with the fracture size ≥25% of the distal articular surface.
