| Objective:1 To investigate the present situation of anti-thrombotic therapy in hospital patients of cardiovascular department with non-valvular atrial fibrillation(NVAF) in the First Affiliated of Chongqing Medical University.2 Whether did the anticoagulant therapy be treated on the NVAF patients,and what kind of medicine they were used.3 Anticoagulant therapy on patients in different degree of stroke risk and bleeding risk.4 The INR monitoring in the hospital NVAF patient who used warfarin.5 The influence factors of anticoagulant therapy.6 The anticoagulant therapy in the special groups combined with NVAF.Method: 1 The hospital patients with NVAF in the First Affiliated Hospital of Chongqing Medical University from January 1, 2012 to December 31, 2014 were recorded by retrospective case analysis. The basic characteristics, the medical treatment plan, and the international standardization ratio were recorded.2 The recording data were analyzed respectively with SPSS20.0 statistical software.3 Patients in special groups combined with AF got individual analysis, and the affection of anticoagulant therapy were evaluated.Results: 1 A total of 21804 hospital patients were treated in the First Affiliated Hospital of Chongqing Medical University from January 1, 2012 to December 31, 2014,and 1390 NVAF patients were enrolled in this study. According to the CHA2DS2-VASc scores, 85.97% of the NVAF patients need anticoagulant therapy. But the actual anti-coagulation from 0 score, 1 score, and more than 1 score were only 41.10%, 36.07%, and 20.42%.2 There were 350 patients(25.17%) taken oral anticoagulants, 347 of which were taken warfarin while 3 patients taken NOAC.3 The patients with non-valvular atrial fibrillation who taken warfarin, the rate of INR is 15.85%.4 According to the study, the age(≥65 years old), coronary heart disease, prior stroke, history of bleeding, may affect the doctor prescribed anticoagulant drugs.5 NVAF patients with CAD, old, prior stroke, history of bleeding, the rate of anticoagulant therapy is 18.59%, 23.18%, 30.57% and11.63%.Conclusion :1 According to CHA2DS2-VASc scores, most patients with atrial fibrillation need anticoagulant therapy, but the rate of anticoagulant therapy is lower than we were expected.2 The patient who were treated anticoagulant therapy had a low rate of INR compliance rate, which did reach the target(INR:2.0-3.0).3 Patients with CAD, old, prior stroke, history of bleeding, can influence the anticoagulant drugs prescription.4 Special groups with atrial fibrillation still needs more guidance of clinical evidence and related guidelines on anticoagulant therapy. |
