| Objective: To study the size and scope of the hole under the subtemporal approach using a minimal access, and guide the using in the clinical.Methods:Four(8 sides) adult cadaverice heads fixed with formalin were used in this study. The horseshoe-like curving posteriorly incision on each side of the cadaverice heads was placed 1 cm anterior to the tragus and start just above the zygomatic arch.A 3-4 cm bone flap was detached with a craniotome.Measure the size of the bone flap and observe the structure with the microscope. Results:The distance of the two ends of the small horseshoe-like curved incision is 55±5mm,the height of the horseshoe-like curved incision is 40±5mm,the size of the bone flap is 3×4cm,the distance from zygomatic arch to the edge of the tentorium is 39.11±1.32 mm,the distance from zygomatic arch to middle of the pons is 45.93±3.52 mm.The basilar bifurcation and its branches can be clearly watched from the side. Conclusion : With this subtemporal approach using a minimal access can easily reach to the basilar bifurcation, without damage the facial nerve and superficial temporal artery. It also can be used in the clinical to clip the aneurysm from the basilar bifurcation and P1. |
