Objective:To investigate the serum levels of WIBG, ZNF706, GABARAPL2 and the hand and wrist joint magnetic resonance imaging in the diagnosis of early rheumatoid arthritis patients and the prognosis.Methods:From March 2014 to December 2015 in 126 cases of joint pain patients in Department of rheumatism of Affiliated Hospital of Chengde Medical College, 61 cases of early stage patients with rheumatoid arthritis(ERA) group, non early rheumatoid arthritis(NERA) group in 34 cases and other connective tissue diseases(CTD) group 31 cases(such as: system lupus erythematosus, Sjogren syndrome, rigidity of ankylosing spondylitis, osteoarthritis, systemic sclerosis, etc.) and normal physical examination of healthy people(healthy) group 39 as control. By enzyme-linked immunosorbent assay(ELISA) to detect the level of serum WIBG, ZNF706, GABARAPL2, and compare.At the same time, wrist and metacarpophalangeal joints scores according to the OMERACT RA MRI scoring system of the 30 ERA patients and 14 CTD patients and 10 cases of normal human hand and wrist MRI international rheumatoid arthritis MRI scoring system(OMERACT-RAMRIS)【 1 】were obtained and compared.All of the MRI images by two high capital Department of Radiology bone imaging diagnosis of doctors diagnosis, respectively and independent reading, when the two disagree, refer to the three senior physicians in the evaluation opinions.Application international rheumatoid arthritis MRI scoring system(OMERACT-RAMRIS) assesses synovitis, bone marrow edema and bone erosion. At the same time to collect all the selected patients general information, such as CRP, ESR, FR and other results. Observation of serum WIBG, ZNF706, GABARAPL2 levels for early diagnosis of rheumatoid arthritis, as well as hand and wrist magnetic resonance imaging for the early diagnosis of rheumatoid arthritis. At the same time, the correlation between laboratory examination and biological marker and magnetic resonance imaging was observed.Result:1 ZNF706,, WIBG and GABARAPL2 in serum of group ERA, group CTD, group Healthy and group NERA.1.1 ERA group, NERA group, CTD group(other rheumatism group) and Healthy(healthy control group) serum ZNF706 levels were [(85.03±2.99),(62.35±2.87),(53.73±3.15) '(47.45±2.55)ng / L], The difference between the 4 groups was statistically significant(P < 0.05), and the level of ZNF706 in group ERA was significantly higher than that in group NERA, group CTD and group Healthy.We see high expression of ZNF706 in patients with early rheumatoid arthritis. In the late stage of rheumatoid arthritis, other rheumatic diseases and healthy people with low expression or no expression, high level of ZNF706 contribute to the early diagnosis of rheumatoid arthritis.1.2 ERA group, NERA group, CTD group(other rheumatism group) and Healthy(healthy control group) serum WIBG levels were [(1973.05±127.35),(1243.99±102.91),(1047.±86.30) '(342.73±29.79)ng /L], The difference between the 4 groups was statistically significant(P < 0.05), and the level of WIBG in group ERA was significantly higher than that in group NERA, group CTD and group Healthy.We see high expression of WIBG in patients with early rheumatoid arthritis. In the late stage of rheumatoid arthritis, other rheumatic diseases and healthy people with low expression or no expression, high level of WIBG contribute to the early diagnosis of rheumatoid arthritis.1.3 ERA group, NERA group, CTD group(other rheumatism group) and Healthy(healthy control group) serum GABARAPL2 levels were [(37.52±2.67),(29.19±1.72),(19.88±0.90)'(14.69±0.86)ng/ml],The difference between the 4 groups was statistically significant(P<0.05), and the level of GABARAPL2 in group ERA was significantly higher than that in group NERA, group CTD and group Healthy.We see high expression of GABARAPL2 in patients with early rheumatoid arthritis. In the late stage of rheumatoid arthritis, other rheumatic diseases and healthy people with low expression or no expression, high level of GABARAPL2 contribute to the early diagnosis of rheumatoid arthritis.2 ERA group, NERA group, CTD group, and Healthy group of hand and wrist joint magnetic resonance imaging score(OMERACTRAMRIS).Application of MRI scoring system(OMERACT- RAMRIS) on the ERA group, NERA group, CTD group, and Healthy group of hand and wrist joint magnetic resonance imaging score. Four score results were 22.11±7.00, 41.90±12.54,1.46 ±1.29, 0.00±0.00,. The difference between the 4 groups was statistically significant(P < 0.05). The magnetic resonance imaging score of the hand and wrist joint was higher In NERA group and ERA group, the score is highest In NERA group,Is lower in CTD group and Healthy group.3 The correlation between ERA group of hand and wrist MRI score and serum markers of WIBG, ZNF706, GABARAPL2 and CRP and ESR and FR.Application of MRI scoring system(OMERACT- RAMRIS) on the ERA group. The correlation between MRI score and serum WIBG level in ERA group was 0.884, and the correlation between znf706 was 0.334, and the correlation between CRP was 0.353, and the correlation between ESR was 0.356, which have statistical significance. No correlation with GABARAPL2 and FR.Conclusion:1 High expression of WIBG, ZNF706 and GABARAPL2 in early rheumatoid arthritis. Biomarkers WIBG, ZNF706, GABARAPL2 are biological markers for early diagnosis of early rheumatoid arthritis.2 NERA patients and ERA patients with hand and wrist joint MRI imaging score higher than the CTD group and Healthy group, MRI imaging score is helpful in the diagnosis of rheumatoid arthritis, and it is helpful for the identification of early rheumatoid arthritis and other arthritis.3 MRI imaging score of ERA patients with has a strong positive correlation with WIBG. High levels of WIBG may be related to the severity of joint destruction.
