Clinical Effect Of Micro-screw Anchorage And Traditional Anchorage Such As Extra-oral Arch: A Systematic Review

Abstract/Summary:
Objective:This study evaluated orthodontic effects of micro-screw anchorage and traditional anchorage like extra-oral arch using a systematic evaluation method, in order to provide references for both clinicians and patients.Methods:In this study, we searched for randomized controlled trials comparing clinical effects of micro-screw anchorage and traditional anchorage. The databases being searched include Pub Med, Cochrane, EMBASE, Ovid, Chinese Periodical Full-text Database, Wanfang Database, Chinese Science and Technology Periodical Database(VIP) and Chinese Biomedical Literature Database. The papers must be published between the establishment of the above databases and November 2015. Rigorous quality assessments and data extractions were made to the qualified studies. Data were analyzed statistically using Rev Man5.3.Results:Eventually 6 studies i.e. 6 clinical trials met the inclusion criteria and 190 patients with malocclusion were included in the study, of which 87 used micro-screw anchorage and 103 used extra-oral archs or other traditional anchorages. Results from meta-analysis showed: 1)micro-screw anchorage is better than traditional anchorages in moving maxillary incisors,improving front teeth protrusion and reducing anterior overbite, with a statistically significant difference [P<0.0001, SMD=0.90, 95% CI(0.56 1.25)]; 2) Compared with traditional anchorages like extra-oral arch, micro-screw anchorage is better in improving the inclination of the maxillary incisors, with a statistically significant difference [P=0.02,SMD=0.52, 95% CI(0.09,0.96)]; 3) Less mesial movement of Maxillary molars is found in micro-screw anchorage group than traditional group, as well as better protrusion improvement [P=0.03, SMD=-0.48,95% CI(-0.90,-0.05)]; 4) In terms of the sagittal change of the skull by maxillary bone, there is no statistically significant different between micro-screw anchorage group and traditional anchorages like extra-oral arch [P = 0.70,SMD=-0.11,95% CI(-0.65,0.43)].Conclusion:Compared to the traditional anchorage group, micro-screw anchorage enables front maxillary teeth to retract more within the indication and has subtle molar advancement,providing a more stable anchorage.
