Objective:Through the comparative analysis of clinical cases, to evaluate the feasibility of orbital septum fat’s reasonable release and re application in lower blepharoplasty. Methods:The 60 clinical cases of elderly patients with lower eyelid skin relaxation, orbital septum fat hernia and combined with nasojugal groove, eyelid buccal groove and crow’s feet were randomly divided into two groups(the study group and the control group).And each group had 30 patients. The study group using: Local anesthesia, skin approach, two layers’ separation of skin and muscle layer [1], infraorbital margin Release of orbital septum, selective release of orbital septum fat according to the different circumstances of the medical treatment, removal of part of the orbital septum fat based on the condition of appropriate, reverse break or transfer of the orbital septum fat which had been released and removed to nasojugal groove, eyelid buccal groove and crow’s feet, and then tighten the orbital septum, orbicularis oculi muscle suspension to strengthen the anterior wall of orbital septum, appropriate removal of excess skin and suture incision. While the control group of patients during the operation, removal of excess orbital septum fat alone, and then tighten the orbital septum and suture for reinforcement, the rational application of orbital septum fat was not used, and other surgical procedures were the same as the study group. Results:In this study, the study group and the control group, a total of 60 patients, follow up for 1~12 months, were evaluated according to the criteria of curative effect[4]. There were no serious complications such as lower eyelid surgery, incision infection after operation in the two groups. 29 patients with palpebral bags, nasojugal groove, eyelid buccal groove and crow’s feet in the study group were obviously improved. The palpebral bags, nasojugal groove and crow’s feet of the only 1 patients in the study group were obviously improved, but the improvement of the buccal groove was less than 49%. While in the control group there were only 24 cases whose palpebral bags, nasojugal groove, eyelid buccal groove and crow’s feet were significantly improved. Through statistical analysis, the effective rate of the study group was significantly higher than that of the control group.
