Study On Hospital Expenditure And Its Influence Factors Of Tuberculosis Patients In Lanzhou

Objectives: To understand the composition of hospital expenditure of tuberculosis patients in Lanzhou City 2014, and explore the influencing factors of tuberculosis patients’ hospital expenditure through the analysis of the tuberculosis patients’ sociological factors, medical institutions’ specific data as well as disease factors. Provide basis for formulating control measures and policies of tuberculosis patients’ hospital expenditure, but also for optimizing tuberculosis prevention funds allocation; proposed possible opinions and suggestions to further reduce the economic burden of tuberculosis patients and improve TB medical expense control management systemMethods: By extracting all the information of patients discharged statistics form in 2014 from Gansu Health and Family Planning Statistical Information and Reporting System, we got 710 cases with active tuberculosis and first hospitalization, and then analyzed the tuberculosis patients’ basic factors, medical institutions’ data, disease factors as well as hospital cost information by using SPSS 18.0 to explore the possible factors affecting the cost of hospitalization, and applied multiple linear regression analysis for comprehensive analysis.Results: 1. Drug costs, comprehensive medical services, diagnostic category costs are the top three in hospitalized tuberculosis patients cost structure, and the differences between all kinds of levels hospitals’ constitute are small. The single factors of patients’ hospital costs found: hospitalization costs differences in gender, admission route and diseases associated with tuberculosis are not statistically significant, but existed in age, occupation, hospital grade, the door(urgent) diagnosis and discharge main diagnostic consistency, admission illness, hospitalization days, surgery, the medical payment and drug costs. 2. Multivariate analysis found that the cost of hospitalization was no significant difference in age, occupation, medical payment, finally, we get the following equation: Y1 = 5.820+0.229Y2+0.102X3-0.316X4-0.153X6+0.367X8+0.406X10Conclusions: 1. The median hospital expenditure of tuberculosis patients in Lanzhou City 2014 is 7047.50 yuan, which makes the economic burden of patients heavy. 2. According to the degree of influence descending order, the main factors of tuberculosis hospital costs are: drug costs, surgery, the door(urgent) diagnosis and discharge main diagnostic consistency, hospitalization days, admission illness, hospital grade. 3. The focus population groups of reducing the disease burden of tuberculosis are rural residents and patients in age 50~age 69. 4. Medical payment is not the influencing factor of tuberculosis patients’ hospital expenditure.
