Objective:Through the observation of clinical effect of modified Er Miao granules on the treatment of HR-HPV persistent infection merged CINI-Ⅱ,confirming the definite effect of modified Er Miao granules in clearing HPV and reversing CINI-Ⅱ, to provide strong support for the further clinical promotion and new drug research and development;Analyzing the expression of P16INK4a in HR-HPV persistent infection merged CINI-Ⅱ,and compared with histopathology and cytology results,to explore the significance in diagnosis and treatment of cervical precancerous lesions.Methods:First select 60 patients who meet the inclusion standard of HR-HPV persistent infection merged CINI-Ⅱ, randomly divided into modified Er Miao granules treatment group 30 cases, interferon control group 30 cases.After 3 months of treatment, the curative effect was observed. Before and after treatment two groups were detected with TCT, HC2, P16INK4a and cervical biopsy under colposcopic guidance. After treatment compared two groups of turning negative rate of HPV, the total efficiency of clearing HPV,reversion of cervical pathological results and p16INK4a expression in cervical lesions in different levels.Results:(1) Turning negative rate of HPV of treatment group is 30.00%,total effective rate of clearing HPV is 73.33%;control group turning negative rate of HPV is 23.33%,total effective rate of clearing HPV was 66.67%. Comparison of two sets of results,However no statistically significant between two groups was found(P=0.440>0.05),but modified Er Miao granules is better than interferon in the clearance of HPV and reduce viral load.(2)In treatment group,18 cases were cured,5 cases were improved,7 cases had no effect;in control group,17 cases were cured,4 cases were improved,9 cases were ineffective.The total effective rate of treatment group in cervical pathological results was 76.67%,the total effective rate of control group was 70.00%.Comparison of two sets of results,P=0.462>0.05,treatment group and control group had no significant difference in reversion of cervical pathological results,but the total effective rate of treatment group is better than the control group.(3) Before treatment, a total of CINI 45 cases, the p16INK4a expression has 19 cases, the positive expression of p16INK4a rate 42.22%; CINⅡ patients with a total of 15 cases, p16INK4a expression has 14 cases, p16INK4a positive expression rate was 93.33%. Before treatment the positive rates of p16INK4a in the treatment group and the control group were 56.67% and 53.33% respectively.After treatment the positive rates of P16INK4a in the treatment group and the control group were 10% and 30% respectively. After treatment, the comparison of two groups of data showed no statistical significance, but modified Er Miao granules treatment group.p16INK4a positive expression rate is far less than interferon control group.Conclusion:In the treatment of HR-HPV persistent infection merged CINI-Ⅱ,modified Er miao granules has a significant clinical effect can not only clear HPV and reduce viral load, also can effectively reverse the cervical intraepithelial neoplasia, it is worth to popularize in the clinical application and further research.There was a significant correlation between the expression of p16INK4a and the grade of cervical lesions.The positive expression rate of P16INK4a in CINI patients was significantly higher than that in CINⅡ patients,and p16INK4a can improve the rate of early diagnosis of cervical cancer, can be used as a important marker of disease progression and prognosis judgment of precancerous lesion of cervix.
