Systemic Lupus Erythematosus(SLE) is an autoimmune disease characterised by severe organ and system damage, complicated clinical symptoms, long course and tending to recurrence. Because the pathogenesis of SLE is complicated, which is confirmed to be correlated with genetic factors, deregulated immune, environment factors and gender, We still do not know it clearly. Modern medical research consider of apoptotic T and B lymphocyte is one of the important cause of SLE. Fas, a main membrane receptor mediated lymphocyte apoptosis, is associated with the pathogenesis of SLE. Traditional Chinese Medicine considers that physical constitution is an important factor for disease occurrence and development. Physical constitution theory has an unique advantages on revealing the pathogenesis of SLE. Combining body constitution theory with the outcomes of modern medicine has an advantage on revealing the pathogenesis of SLE and laying more scientific theoretical basis for prevention and treatment with Traditional Chinese Medicine for SLEObjectiveTo explore the pathogenesis of systemic lupus erythematosus on Traditional Chinese Medicine, we compare the Fas expression in peripheral blood T and B lymphocytes and the percentage of their apoptosis in patients with Systemic Lupus Erythematosus classified into weak physical constitution and strong physical constitution, which provide a more scientific theory basis for prevention and treatment for SLE with traditional Chinese medicine.MethodsAccording to the standard of classification of SLE patients,46 cases of patients with Systemic Lupus Erythematosus from the first affiliated hospital of guangzhou university of TCM department of rheumatism outpatient and inpatient during the period from August 2015 to February 2016 are investigated, including the items of name, age, sex, the time of illness etc. The cases are classified into weak physical constitution including qi deficiency, yang deficiency, yin deficiency type and strong physical constitution including phlegm-dampness, blood stasis, damp-heat type, according to the criterion which is established On April 9,2009. The cases with specific endowment physical constitution and Peace physical constitution are not researched. Each patient is evaluated Lupus disease activity by SLEDAI index. Venous Blood of each patient was analyzed by using flow cytometry to detect the percentage of Fas expression on peripheral blood T and B lymphocytes and the percentage of their apoptosis. The results are analyzed by SPSS22.0 software for statistical processing. We compare the percentage of Fas expression in peripheral blood T and B lymphocytes and the percentage of their apoptosis in patients with Systemic Lupus Erythematosus of weak physical constitution group and strong physical constitution group. The percentage of Fas expression in peripheral blood T and B lymphocytes, the percentage of apoptosis, the level of ds-DNA, SLEDAI indexs are analyzed by correlation analysis.Results1.The percentage of apoptotic lymphocytes in peripheral blood issignificantly increased in weak physical constitution compared to strong physical constitution.2. There are positive correlation between T lymphocytes Fas expression in peripheral blood and lupus disease activity or anti-bodies to double stranded DNA.3. In active period of SLE, Fas expression in B lymphocytes of peripheral blood has a positive correlation with B lymphocyte apoptosis rates.Conclusion1. The present studys have showed a signicantly higher level of apoptotic T lymphocyte in peripheral blood of SLE patients compared with normal healthy control, which reflects the defects in the clearance of apoptotic. This is considered to be closely related with the Pathogenesis of SLE. Comparing with the individuals with strong physical constitution, the individuals with weak physical constitution may be more likely to develop SLE.This conclusion is identical with the pathogenesis that qi-insufficiency and congenitally deficiency of SLE in Traditional Chinese Medicine. Therefore, the weak physical constitution is a root cause of developing SLE.2. There are positive correlation between T lymphocytes Fas expression in peripheral blood and lupus disease activity or anti-bodies to double stranded DNA. The Fas expression in peripheral blood T lymphocytes has important value in evaluation of lupus disease activity.3. The apoptosis pathway mediated by Fas may be the main pathway of apoptotic B lymphocyte in peripheral blood in active stage of SLE.
