Objective:Applying the theory of traditional Chinese medicine combined with clinical practical implementation of auricular point acupressure in patients with knee osteoarthritis who need to take a unilateral total knee arthroplasty (TKA). To observed the analgesic effect in postoperative TKA, aiming to provide patients of TKA a simple,effective and safe adjuvant analgesia.Method:Sixty patients who had undergone a TKA were randomly assigned to the observation group and the sham control group, thirty patients in each group. To apply auricular point acupressure in the observation group on the base of the routine PCA,and to apply sham auricular point acupressure in the control group on the base of the routine PCA.A visual analog scale(VAS) was used to assess rest and moving state pain intensity at different time points after surgery. The medication consumption of PCA,the total frequency of use of PCA and the time of the first use of PCA were recorded,and the knee range of motion was measured at 24,48,72h and 1 week after surgery. The KSS scores wree recorded at 1 week,l and 3 month after surgery.Nausea,vomiting,dizziness,sleepiness,urinary retention and other adverse effects were recorded.Result:The VAS scores in observation group at rest after 12,24,48,72h of operation were less than that in control group(P<0.05).The difference of the rest VAS scores after 6h of operation was not statistically significant(P>0.05).The VAS scores in observation group at moving state after 24,48,72h of operation were less than that in control group(P<0.05). The amount of drug use of PCA after 1,2,3d of operation and the total frequency of use of PCA in observation group were less than that in control group(P<0.05).The time of the first use of PCA in observation group was more than that in control group(P<0.05).No use of cloth flunarizine hydrochloride injection in two groups.The incidence of nausea and vomiting in observation group were less than that in control group(P<0.05).The differences of the KSS scores after 1 week,1 and 3 month of operation were not statistically significant(P>0.05).The knee range of motion in observation group after 24,48,72h of operation were more that in control group(P<0.05),but no difference after 1 week of operation(P>0.05).Conclusion:Auricular point acupressure can effectively alleviate the early postoperative pain,reduce the amount of drug use of PCA and delay the time of the first use of PCA,effectively improve the knee range of motion,and reduce the incidences of nausea and vomiting. Applying auricular point acupressure in the treatment of pain in the postoperative period of TKA is a simple,effective,safety and low cost,and which has obviously postoperative analgesic effect,and can contribute to the recovery of knee joint function.
