The Epidemiological Features Andcontrol Status Of Hypertension In Guangxi Autonomous Region’s 9 Areas,2015

Objective:To investigate the epidemiological features of hypertension and the factors affecting the hypertension control, doing effect to form the strategies of preventing and controlling hypertension. Method:A strafied multistage random sampling method was used to select a representative sample tribe of 17100 dwellers (lived more than 6 months) aged 15 or over from 9 areas in GuangXi:including YongFu, LiuJiang, LaiBin, YuLin, WuMing, BaMa, QinZhou, GuiPing, YongNing. Used questionnaires to gain informations and measured height, weight, waist and blood pressure, then analysed the data to find out the prevalence, awareness, treatment and control of hypertension, estimated the factors associated with the prevalence and management of hypertension. Result:①The total number of participants was 17100 from 9 areas across the whole GuangXi,and 16559 of them completed the survey, among whom aged 15 to 24 years were 2173 (13.12%), aged 25 to 34 years were 2842 (17.16%), aged 35 to 44 years were 3045 (18.39%), aged 45 to 54 years 2579 (15.57%), aged 55 to 64 years were 2248 (13.58%), aged 65 to 74 years were 1935 (11.69%), age 75 years throught the oldest were 1755 (10.49%). ② Physiological index of male dwellers:Mean age was 46.23±19.48 years old, mean height was 1.64±0.07m, mean weight was 59.82±9.62Kg, mean waist circumference was 79.75±9.16cm, mean BMI was 22.21±3.38Kg/m2, mean heart rate was 76.77±9.39 beats/min and mean VAI was 7.52±6.67; Mean SBP was 127.90±18.37mmHg, among them the highest area was YongNing district of NanNing (131.81±20.12mmHg), showed no difference with GuiPing and YongFu, but obviously higher than the rest of 6 areas (P<0.05), the lowest area was QinZhou (122.76±15.60mmHg), showed no difference with YuLin and LiuJiang while obviously lower than the rest of 6 areas (P<0.05). Mean DSP was 75.87±9.69mmHg, among them the highest area was GuiPing (77.73±9.41mmHg), showed no difference with YongFu and LiuJiang, but obviously higher than the rest of 6 areas (P<0.05), the lowest area was WuMing (73.23±9.98mmHg), obviously lower than the rest of 8 areas (P<0.05). Physiological index of female dwellers:Mean age was 47.51±19.36 years old, mean height was 1.53±0.06m, mean weight was 51.49±8.94Kg, mean waist circumference was 77.01±9.63cm, mean BMI was 22.07±3.44Kg/m2, mean heart rate was 77.43±9.78 beats/min and mean VAI was 5.57±4.58; Mean SBP was 126.11±20.00mmHg, among them the highest area was YongNing district of NanNing (129.25±22.45mmHg), showed no difference with GuiPing, YongFu and LiuJiang, but obviously higher than the rest of 5 areas (P<0.05), the lowest area was WuMing (123.26±20.42mmHg), showed no difference with LaiBin, QinZhou, YuLin and BaMa while obviously lower than the rest of 4 areas (P< 0.05). Mean DSP was 73.36±9.76mmHg, among them the highest area was GuiPing (74.66±10.24mmHg), obviously higher than WuMing and LaiBin, but showed no difference with the rest of 6 areas (P<0.05), the lowest area was WuMing(68.71±9.52mmHg), obviously lower than the rest of 8 areas (P< 0.001). Mean SBP of male dwellers was higher than that of female dwellers (127.90±8.37mmHgVS 126.11±20.00mmHg; P<0.001) and mean DBP was also higher among men than women (75.87±9.69mmHgVS 73.36±9.76mmHg; P < 0.001). ③ Prevalence of Hypertention:Among all 16559 eligible participants:3835 people were hypertensive, the prevalence of hypertension was 23.2%, the age-adjusted prevalence was 16.9%. There were 7878 male dwellers, 1822 people of them were hypertensive, the prevalence of hypertension was 23.1%, the adjusted prevalence was 17.6%; And 2013 people of all 8681 female dwellers were hypertensive, the prevalence of hypertension was 23.2%, the adjusted prevalence was 16.4%, no obvious difference of prevalence between men and women was showed (P>0.05). Over all 9 areas investigated, people from YongNing district of NanNing city had the highest prevalence of hypertension (22.2%), which was obvious higher than the rest of 8 areas (P< 0.05); And people from QinZhou had the lowest prevalence of hypertension (13.7%), showed no difference with YuLin and LaiBin while obviously lower than the rest of 6 areas (P<0.001).The prevalence of hypertension showed an increasing trend between both men and women while they were growing older, and it became more obvious when aged 45 or over, men had a higher prevalence of hypertension before aged 65, and it were lower than that of women when aged 65 or over, the prevalence of hypertension was higher than 50% among both men and women when aged 65 or over (50.77% VS 53.94%), the people of both men or women aged 75 or over had the highest prevalence of hypertension (58.79% VS 63.53%).④ Awareness of Hyppertension:Among all 3835 hypertensives,1350 people of them knew their conditions, the awareness of hypertension was 35.2%, the age-adjusted awareness was 18.9%. Male patients had lower awareness of hypertension compared to female patients (32.8% and adjusted 17.2% VS 37.4% and adjusted 20.1%; P<0.05). Male patients aged 65 to 74 years had the highest awareness of hypertension while aged 35-to 44 years were the lowest (40.78% VS 13.82%), female patients aged 55 to 64 years had the highest awareness of hypertension while aged 35 to 44 years were the lowest (42.4% VS 25.25%). Over all 9 areas investigated, people from YongFu had the highest awareness of hypertension (27.2%), showed no difference with QinZhou while obviously higher than the rest of 7 areas (P<0.05). And people from LaiBin had the lowest awareness of hypertension (7.6%), which was obvious lower than the rest of 7 areas except for BaMa (P<0.05). ⑤ Treatment of Hypertension:Among all 3835 hypertensives,1430 people of them had ever taken antihypertensive medicines, the treatment of hypertension was 36.6%, the age-adjusted treatment was 19.6%, Male patients had lower treatment of hypertension compared to female patients (17.7% VS 21.0%; P<0.05). The people of both men or women aged 45 to 54 years had the highest treatment of hypertension (40.61% VS 46.93%), male patients aged 15 to 24 years had lowest treatment (2.04%), and female patients aged 35 to 44 years were lowest (26.73%). Over all 9 areas investigated, people from YongFu had the highest treatment of hypertension (28.7%), showed no difference with QinZhou while obviously higher than the rest of 7 areas (P<0.05). And people from BaMa had the lowest treatment of hypertension (10.0%), which was obvious lower than the rest of 7 areas except for LaiBin (P<0.05). ⑥ontrol of Hypertension: Among all 1430 patients who had ever taken antihypertensive medicines,350 people of them had proper blood pressure lower than 140/90mmHg, the control of hypertension was 24.9%, the age-adjusted treatment was 18%, the adjusted control of hypertension showed no difference between men and women (17.6% VS 19.0%; P>0.05). Male patients aged 45 to 54 years had the highest control of hypertension while aged 55 to 64 years were the lowest (33.33% VS 23.53%), female patients aged 35 to 44 years had the highest control of hypertension while aged 75 years or over were the lowest (40% VS 19.92%). Over all 9 areas investigated, people from YuLin had the highest control of hypertension (36.1%), showed no difference with YongFu and WuMing while obviously higher than the rest of 6 areas (P<0.05). And people from LaiBin had the lowest control of hypertension (4.1%), which was obvious lower than YuLin and YongFu (P< 0.05), but showed no difference with the rest of 6 areas. Conclusion:Throught the epidemic survey on hypertension from 9 areas of GuangXin, we found that prevalence of hypertension in GuangXi was 16.9%, prevalence between men and women showed no difference(17.6% VS 16.4%), geographic variation in prevalence of hypertension among 9 areas of GuangXi showed north higher than south, and it showed a increasing trend while age grew. Awareness of hypertension was 18.9%, female had higher awareness than male, people aged 55 to 74 years had the highest awareness of hypertension while aged 35 to 44 years were the lowest. Treatment of hypertension was 19.6%, female had higher treatment than male, people of both men or women aged 45 to 54 years had the highest treatment of hypertension, male patients aged 15 to 24 years had lowest treatment, and female patients aged 35 to 44 years were lowest. People from YongFu had the highest treatment of hypertension (28.7%) and people from BaMa had the lowest treatment of hypertension (10.0%).Control of hypertension was 18%, which showed no difference between men and women. Male patients aged 45 to 54 years had the highest control of hypertension while aged 55 to 64 years were the lowest, female patients aged 35 to 44 years had the highest control of hypertension while aged 75 years or over were the lowest. Over-all 9 areas investigated, people from YuLin had the highest control of hypertension and people from LaiBin had the lowest control of hypertension. All these result demonstrated that:GuangXi had a lower prevalent of hypertenion compared with the whole China, yet awareness, treatment and control of hypertention is severely lower than these of the whole nation and differed from area to area, more efficient strategies of preventing and managing hypertention in GuangXi was needed.
