Objective:To investigate the effectiveness and influencing factors of 131I treatment for elderly (age≥60 years) patients with differentiated thyroid carcinoma (DTC).Methods:Seventy-two postoperative elderly patients with DTC (41 females,31 males; mean 67.2±5.9 years) who received 131I treatment were analyzed retrospectively. All patients were followed up for more than 6 months, and the effectiveness of 131I treatment was evaluated by radioiodine whole body scanning, serum Tg level and imaging examination. To identify relevant influencing factors of 131I treatment including gender, age, surgical method, pathological type, tumor size, number of tumor foci, capsular invasion, cervical lymph node metastases, distant metastases, family history and basic diseases, univariate and multivariate analysis were performed.Results:Of included patients, the rates of complete remission, partial response, invalid and total effective rate of radioiodine treatment were 27.8%(20/72),43.0%(31/72),29.2%(21/72) and 70.8%(51/72) respectively during follow-up. Univariate logistic regression analysis showed that, there was a significant correlation between the therapeutic effectiveness of 131I in elderly DTC and age, number of tumor foci, capsular invasion, cervical lymph node metastases and distant metastases (P<0.05), but the correlation was not significant with gender, surgical method, pathological type, tumor size, family history and basic diseases (P>0.05). Multivariate logistic regression analysis showed that age^70 years (P=0.015), mulifocality (P=0.030), cervical lymph node metastases (P=0.040) and distant metastases (P=0.006) were independently influencing factors for therapeutic effectiveness of 131I treatment.Conclusions:131I treatment is an effective method for postoperative elderly patients with DTC. The effectiveness of 131I treatment was related to patient’s age, number of tumor foci, cervical lymph node metastases and distant metastases. Thus, investigation of relevant influencing factors is required to timely adjust the therapeutic strategy and improve the effective of 131I treatment in elderly patients with DTC.
