Objective:To explore the disease features, clinical characteristics and pregnancy outcomes of maternal near miss cases in ICU and summarize the treatment experiences of obstetrics associated with ICU in saving maternal near miss,in order to provide clinical data and measures for reducing the morbidity and mortality of maternal and the newborn,improving successful salvage rate.Methods:The clinical data for 53 ceases of maternal near miss obstetrics patients who were transferred from the Department of Obstetrics to Intensive Care Unit (ICU) at the the First Affiliated Hospital of Guangxi Medical University during a 3-year period (2013.01-2015.12) were retrospectively analyzed. Through grouping comparison,the main reasons and pregnancy outcomes for ICU admittance of maternal near miss were summarized.Results:The overall rate of admission into ICU was 0.39%.2.The main factor for admission to an ICU was obstetric reasons,and obstetric hemorrhage, hypertensive disorders in pregnancy and cardiovascular diseases in pregnancy were the most frequent obstetric diseases for admission to an ICU.3.Compared with non obstetric factors group,patients from obstetric factors group were more likely to be admitted to an ICU, and they occupied a higher bleeding volume and blood transfusion rate.Usually,their hospital stay in ICU was shorter, operation time was longer and the rate of conducting hysterectomy is higher.However,there were no significant difference in age, gestational age, total hospitalization expenses, hospitalization time, prenatal examination, anesthetic methods,ways of termination of pregnancy, neonatal sex, birth weight and neonatal asphyxia between the two groups 4.Pregnancy outcomes:there were 0 cases of maternal death with a death rate of 0%. The umber of deliveries among the 53 cases was 59,which included 5 cases of labor induction,4 cases of dead fetus and 50 cases of live births with a perinatal mortality rate of 8%.Conclusions:1.Obstetric treatment combined with ICU co-treatment of critically ill pregnant women is a new idea and an important measure of modern obstetrics emergency.It has great significance in ensuring safe motherhood and reducing mortality rates.2.Critically ill pregnant women who are admitted to an ICU mainly due to obstetric factors, especially after delivery.3.The most frequent obstetric reason for admission to an ICU are obstetric hemorrhage,hypertensive disorders in pregnancy and cardiovascular diseases in pregnancy.4.The establishment of obstetric ICU is a modern obstetric trend.
