The Relationship Between HBV PreS,PreC/BCP Mutation And HCC And Establishment Of Consensus Sequence Of HBV In HCC High Incidence Areas In Fusui County

Objective:To investigate the relationship between hepatitis B virus(HBV) PreS,PreC/BCP region mutations and HBV-related hepatocellular carcinoma (HCC),and to establish consensus sequences of PreS,S and PreC/BCP gene of HBV in Fusui county.Methods:1.30 HBV-related HCC patients and 35 asymptomatic HBV carriers were selected from hepatocellular carcinoma high incidence areas in Fusui county. DNA sequences of HBV PreS and PreC/BCP region were amplified by nsted-PCR,S region was amplified by ordinary PCR,then sequenced directly. Evalua-ing the association between HBV gene mutations and the risk of HCC from traditional case-control study and nested case-control study.Observing PreS deletions,PreS2 start codon mutation, T53C,C3026T,T1858C,A1775QG1896A and A1762T/G1764A.2.Gene of PreS、S and PreC/BCP in 89 asymptomatic carrier were sequenced and genetype were determined by homology analysis. Consensus sequences of HBV prevailing in Fusui county were established base on diffirent genetypes.Results:1. A1775G,T1858C,PreS deletion had statistical significance difference, among which,A1775G was higher in control group rather than in HCC group, odds ratio(OR) are 0.215、3.636、4.306.2.The longitude study demonstrated that A1762T/G1764A、T1858C、PreS2 start codon mutationsand, C3026T、G1896A and PreS deletion occured 4-5 years before HCC.Meanwhile,T53C occurd 2-3years befor HCC.3.Nested case-control study showed,T1858C, PreS deletion,the lack of A1775G had statistical significance difference in HCC group,odds ratio are 2.877,4.267,3.706。4.Compared with same genetype HBV sequences of other parts of China recorded in Gen Bank, the consensus sequences of Fusui County are of great difference.The difference of consensus sequences between different genetypes in Fusui County shows, this difference is bigger than that of different areas but with same genetype.Conclusions:1. A1775G,T1858C,PreS deletion contributed to the occurrence of HCC,while A1775G mutation was a protected factor to HCC.2. A1762T/G1764A、T1858C、PreS2 start codon mutationsand, C3026T、 G1896A and PreS deletion reacted on the early stage during the advance of HCC,the disappearance of the protected factor,A1775G, would prompting the diagnoses of HCC,which could be a predictive marker for HCC.3.The consensus sequences of PreS、S and PreC/BCP gene of HBV in Fusui county were established,which would lay foundation for study of genetic mutations in Fusui county in the future.
