ObjectiveIn order to evaluate the effectiveness and safety of this therapy system, to discuess its possible mechanism in structure. Then to supply more completed theory for the therapy system, and provides a feasible conservative treatment for treating degenerative lumbar spinal stenosis.Methods(1) Clinical research:58 cases meet the inclusion and exclusion criteria who has suffered degenerative lumbar spinal stenosis and treated by orthopedics treatment department during March 2015 to December 2015. Of which 26 were male and 32 were female, the average age was 66.38±9.26 years old, ranging from 47 to 80 years; the average Course of disease was 5.67 years from 4 months to 20 years. Group divided:there are 29 patients belong to the Lumbar rolling movement with forward bending group and 26 of them complete the follow-up.29 people were conventional therapy group while 25 of them finish the follow-up. Two groups were compared with sexual, age, length of the history data and show no significant difference(P<0. 05).2 cases of the Lumbar rolling movement with forward bending group were unable to return on time and remove,3 cases was lost due to the contact loss, lease Relapse and go to the other hosipital,1 case take the operation during the follow-up period-At last 46 of 53 cases have completed the follow-up successfully. Compared the two group with VAS scores, JOA scores, ODI scores, ICD and also the recurrence during the period. Statistical analysis the data with SPSS 20.0 software.(2) Radiological analysis:Choose volunteers who are consistent with the degenerative lumbar spinal stenosis and meeting the requirements of this subject. Let them take Lumbar CT scan, use the Digimizer Software to measure, analysis and calculate the images. Evaluating the correlation between effects of Lin flexion rolling correlation of clinical curative waist method in treatment of degenerative lumbar spinal stenosis and different types of stenosis of DLSS regarding Spearman rank correlation analysis. deal with the SPSS 20.0 software.Results(1)Clinical research:The statistical analysis results:both the Lumbar rolling movement with forward bending group and the conventional therapy group can achieve a significant effect in clinic(P<0.05); The clinical curative effect between the Lumbar rolling movement with forward bending group and the conventional therapy group shows no significant differences (P >0.05); the Improvement of intermittent claudication distance between two groups shows significant differences (P<0.05); the recurrences and adverse reactions between two groups shows significant differences(P<0.05)(2) Radiological analysis:29 cases were included in the case, and successful completion of analysis. The baseline data of contrast, its narrow sub type and included patients age, sex and when incorporated into clinical efficacy indexes (JOA and vas,ODI, the farthest claudication distance) no correlation. Regarding the patients with mild to moderate DLSS, the patients were divided into the central canal stenosis, the intervertebral foramen stenosis and lateral recess stenosis according to the lumbar imaging data. There were no significant differences in the baseline characteristics (gender, age) of the 29 patients (P>0.05). Comparative analysis of three narrow minute before and after treatment in patients regarding JOA, VAS and ODI scores, the farthest claudication distance (P<0.05). Using Spearman correlation analysis, the difference was statistically significant between the groups of the central canal stenosis, the intervertebral foramen stenosis and the lateral recess stenosis type groups. On three narrow minute efficacy index for pairwise comparison was between the three groups VAS, JOA and the farthest claudication distance ameliorate differences had no statistical significance (P> 0.05); central canal stenosis and intervertebral foramen stenosis type grouping between ODI score improvement rate had no significant difference (P>0.05); lateral recess stenosis and central tube type of stenosis and intervertebral foramen stenosis between groups differences have statistical significance (P>0.05).ConclusionUsing the Lumbar rolling movement with forward bending is effective in the treatment of degenerative lumbar spinal stenosis and control its recurrence. By improving the height and area of the intervertebral foramen to produce a therapeutic effect may be the mechanism. The roller Lin flexion waist treatment applicable to central tube type of stenosis and intervertebral foramina and lateral recess type DLSS patients, especially the application in the central canal type and intervertebral pass type patients.
