ObjectiveTo investigate clinical effects of Traditional Chinese Medicine on gastric low-grade intraepithelial neoplasia (LIN) based on the idea of preventive treatment and provide clinical evidence for Traditional Chinese Medicine treatment on this disease.Methods(1)Clinical data of 137 patients with LIN confirmed by endoscopic biopsy in Shandong Provincial Hospital affiliated to Shandong University from September 2013 to December 2014 were collected. All the patients were divided into 2 groups at random, the control group (33 cases) and Chinese herbal observation group (observation group blow,104 cases).(2)The observation group once more divided into 3 groups according to different types, among which,35 cases in the pattern of stagnation of liver Qi,40 cases in the pattern of dampness heat in spleen and stomach,29 cases in the pattern of gastric blood stasis type. They were given to chaihu shugan powder, lianpo decoction, danshen decoction and shixiao powder. Control group taking rebamipide tablets, each one, three times a day. Both group were treated for 3 months.(3)A11 patients were followed up for 9 months. The clinical symptoms, endoscopic and pathological changes were compared and analyzed after 3 months treatment and 9 months follow-up of the two groups. Then evaluated the efficiency of Traditional Chinese Medicine based on syndrome differentiation in LIN.(4)Observe endoscopic features in different syndromes patients.Results(1) Clinical symptom score:After 3 months treatment, the score of each type of the observation group and control group were7.00±4.42、6.55±3.94、6.54±4.08 and 9.67±3.78, significant difference between three types of the observation group and control group was observed(P< 0.05); No significant difference was observed between three types of the observation group(P>0.05); All types of the observation group had significant difference when compared with prior treatment(P<0.05); Control group had significant difference when compared with prior treatment(P< 0.05). After 9 months follow-up, the score of each type of the observation group and control group were8.11±3.68、7.68±3.68、7.43±3.70 and 10.93±3.67, the comparison was of evident statistics significance (P<0.05); No significant difference was observed between three types of the observation group(P>0.05); All types of the observation group had significant difference when compared with prior treatment (P <0.05); Control group had no significant difference when compared with prior treatment(P>0.05). There was no significant difference when compared 9 months follow-up and 3 months treatment in all groups (P>0.05).(2) Clinical symptom effective rate:After 3 months treatment, the rate of each type of the observation group and control group were88.57%、92.11%、85.71% and 60.00%, significant difference between three types of the observation group and control group was observed(P< 0.05); No significant difference was observed between three types of the observation group(P>0.05). After 9 months follow-up, the rate of each type of the observation group and control group were77.14%、81.58%、 75.00% and 46.67%, the comparison was of evident statistics significance(P<0.05); No significant difference was observed between three types of the observation group (P>0.05). There was no significant difference when compared 9 months follow-up and 3 months treatment in all groups (P>0.05).(3) Gastroscope performance effective rate:After 3 months treatment, the rate of each type of the observation group and control group were82.86%,89.47%,82.14% and 56.67%, significant difference between three types of the observation group and control group was observed(P< 0.05); No significant difference was observed between three types of the observation group (P>0.05). After 9 months follow-up, the rate of each type of the observation group and control group were74.29%,73.68%, 75.00% and 46.67%, the comparison was of evident statistics significance (P<0.05); No significant difference was observed between three types of the observation group (P>0.05). There was no significant difference when compared 9 months follow-up and 3 months treatment in all groups (P>0.05).(4) Pathological change effective rate:After 3 months treatment, the rate of each type of the observation group and control group were88.57%,81.58%,82.14% and 50.00%, significant difference between three types of the observation group and control group was observed(P< 0.05); No significant difference was observed between three types of the observation group (P>0.05). After 9 months follow-up, the rate of each type of the observation group and control group were71.43%,71.05%, 71.43% and 40.00%, the comparison was of evident statistics significance (P<0.05); No significant difference was observed between three types of the observation group (P>0.05). There was no significant difference when compared 9 months follow-up and 3 months treatment in all groups (P>0.05).(5) Comprehensive therapeutic effect:After 3 months treatment, the rate of each type of the observation group and control group were85.71%,86.84%,82.14% and 56.67%, significant difference between three types of the observation group and control group was observed(P< 0.05); No significant difference was observed between three types of the observation group (P>0.05). After 9 months follow-up, the rate of each type of the observation group and control group were74.29%,73.68%, 75.00% and 43.33%, the comparison was of evident statistics significance (P<0.05); No significant difference was observed between three types of the observation group (P>0.05). There was no significant difference when compared 9 months follow-up and 3 months treatment in all groups (P>0.05).(6) No obvious side effects of the two groups were found in the research.(7)Patients with different syndromes had different endoscopic manifestations.The pattern of stagnation of liver Qi:Gastric mucosal erythema can be points or flake, biliary and duodenal fluid often flows into gastric cavity, mucus lake mostly yellowish. The pattern of dampness heat in spleen and stomach:Gastric mucosal often has hyperemia and edema, erosion can be presented like piebald. The pattern of gastric blood stasis type:Gastric mucosal has granular or nodular, submucosal vascular can be seen easily, sometime accompanied with old hemorrhage.Conclusion(1)Traditional Chinese Medicine based on syndrome differentiation treatment can significantly improve the clinical symptoms of patients.(2)Traditional Chinese Medicine based on syndrome differentiation treatment can significantly improve the endoscopic manifestations of patients.(3)Traditional Chinese Medicine based on syndrome differentiation treatment can significantly improve the pathological results of patients.(4)Traditional Chinese Medicine based on syndrome differentiation treatment can significantly improve the total clinical effective rate of patients.(5) Patient with different syndromes had different endoscopic features.(6) Traditional Chinese Medicine based on syndrome differentiation treatment have stable and safety long-term clinical efficacy, blocking progress in gastric low-grade intraepithelial neoplasia.(7)The idea of preventive treatment has a positive guiding significance in gastric low-grade intraepithelial neoplasia.
