Objective:To report a detailed case of levofloxacin inducing severe gastrointestinal adverse drug reaction, review relevant literatures, and to improve the understanding of levofloxacin inducing gastrointestinal adverse drug reaction.Methods:Reporting a detailed case of levofloxacin inducing severe gastrointestinal adverse drug reaction. Retrospectively analyze 7451 cases published since 1996 through searching the Wanfang Database and PubMed.Result:The patient had nausea, vomit and poor appetite during the treatment of acute exacerbation of chronic bronchitis. The symptoms of nausea and vomit were progressing after the treatment. After stopping the levofloxacin treatment because of insomnia, the patient’s gastrointestinal symptom disappeared. Rulling out other possible primary disease which can cause gastrointestinal symptoms, we confirmed that levofloxacin caused the gastrointestinal adverse reaction. This study collected 7451 cases with adverse reaction caused by the treatment of levofloxacin. The adverse effect of central and peripheral nervous disorders was the most common ones with the percentage of 27.10% among the 7451 patients. The second one is skin and appendages disorders with the percentage of 22.70%. There are 1324 patients with the side reaction of gastrointestinal symptoms. The total incidence of the three adverse reactions was 67.57% of the total cases.44.31% of the 4431 patients was the elderly (≥60 years old) patients.4109 cases (5120 cases) were treated with levofloxacin through intravenous way. About 78.32% of the patients suffered from adverse drug reactions within three days.6581 of the 6632 patients recovered after withdrawing the drug. Howeve,34 cases died because of the adverse reaction.Conclusion:Levofloxacin can lead to severe gastrointestinal adverse reactions, which is easy to be overlooked. Clinical doctors should pay more attention to the adverse reaction of gastrointestinal disorders caused by levofloxacin.
